Middleton 3440 Westwood Avenue Enoch, Utah 84721 Subject:Plan Approval, Well Drilling, John Middleton Well; Private Well, General Iron County System 11000, File #14164 Dear John Middleton:The Division of Drinking Water (the Division) received the plans and specifications for the proposed drilling of the John Middleton Well from Chris DeKorver, P.G., Bowen Collins & Associates, on April 22, 2024. A Preliminary Evaluation Report (PER) was also submitted at the same time. After comments and revisions of the PER had been resolved, concurrence was able to be issued, which is included later in this letter. The drilling process consists of a 32-inch diameter surface borehole to a depth of 120 feet. A 24-inch diameter permanent surface casing will also be installed to a depth of 120 feet. The well will be grouted between the borehole and surface casing to a depth of 120 feet with neat cement grout. The John Middleton well will be drilled to a total depth of approximately 700 feet with a minimum 23-inch diameter well borehole and a 16-inch diameter production well casing and screens at various depths. The surface casing (24-inch) will be a 0.3125-inch wall, new steel meeting AWWA Standard A-100 or ASTM A53 Grade B. The blank steel casing (16-inch) will be mild steel meeting AWWA Standard A-100 or ASTM A53 Grade B steel with 0.3125-inch thickness to a depth of approximately 400 feet. The screens will be 16-inch HSLA steel Ful-Flow Louvered, 50 slot screens from 400 feet to 700 feet deep. 8X12 SRI or equivalent gravel will be placed between the permanent production casing and the borehole from the ground surface to 700 feet Below Ground Surface (BGS). A steel ring will be welded at the top of the casing between the 24-inch diameter surface casing and the 16-inch diameter production casing to seal the inner annular space where the gravel pack will be installed.The project also includes well development, logging, and pump tests. We have received the following information for John Middle Well Drilling: Plans and specifications for borehole and well drilling. Preliminary Evaluation Report. Valid Start Card (or authorization to drill letter) from the Division of Water Rights. We have completed our review of the plans and specifications, stamped, and signed by Chris DeKorver, P.G., and dated April 18, 2024, and found they basically comply with the applicable portions of Utah’s Administrative Rules for Public Drinking Water Systems. On this basis, the plans for drilling the John Middleton Well are hereby approved. This approval pertains to well drilling, development, aquifer testing, and disinfection of the John Middleton Well only. Please be aware that discharge permits may be required by Utah Division of Water Quality for discharges generated during well drilling and aquifer drawdown testing. The Utah Division of Water Rights (State Engineer’s Office) regulates the drilling of water wells. Before the drilling of a well commences, the well driller must receive a start card from the State Engineer’s Office. Please be aware that, for public drinking water supply wells, in addition to the Division of Drinking Water’s R309-515 rule, the Division of Water Rights’ rule R655-4 also applies and shall be followed. After drilling is completed, if this well is ever to be used in a Public Water System (PWS), you are required to submit additional information outlined in R309-515-6(5)(b) and (c) for review andobtain an approval from the Director for equipping this well and constructing discharge piping and infrastructure necessary for introducing the well water into the distribution system. After obtaining the well equipping approval, you’ll then be required to obtain an Operating Permit before the John Middleton Well may be put in service in a PWS. A checklist outlining the well approval process, including the items required for well equipping and an Operating Permit, is enclosed for your information. Approvals or permits from the local authority or county may be necessary before beginning construction of this project. As the project proceeds, notice of any changes in the approved design, as well as any change affecting the quantity or quality of the delivered water, must be submitted to the Division. We may also conduct interim and final inspections of this project. Please notify us when actual construction begins so that these inspections can be scheduled. This approval must be renewed if construction has not begun or if substantial materials have not been ordered within one year of the date of this letter. Preliminary Evaluation Report Concurrence The Division received the Preliminary Evaluation Report (PER) for this source from your consultant, Chris DeKorver, P.G., with Bowen Collins & Associates, on April 22, 2024. The Division requested additional information via email on May 1, 2024. Your consultant provided a signed statement for use in lieu of land use agreements on May 21, 2024. The Division concurs with this report. This report must be refined, and a complete Drinking Water Source Protection (DWSP) plan submitted within one year of the date of this letter.
