And I do thank you all for what you do and trying to help get us there and I never seem to finish the follow up. But I do push to correct the deficiencies if that's any consolation. Attached are the pictures taken of the items after a phone call with Tom on how to address the flooding and the solenoid leaking. We dug out around the pump house to better expose the height of the pumphouse floor and how it slopes out and around it.The picture with the doorway is on the north side, the east side measurement and a view from the south side looking north to try and show the fall around the building. I don't know how well that really comes through in the picture so if you need more or different views let me know. We had Cla-Val refit the entire valve in and replace the rubber components and ensure it was operating properly. I was never sent their invoice for documentation so if you would need more than the picture please let me know as I will have to track it down from the HOA. 6/6/24, 9:06 AM State of Utah Mail - Fwd: 29107 Pole Patch Compliance https://mail.google.com/mail/b/AEoRXRS0Tq07SjrdoXVFrrL6QBpFhfM9MnHU6JpB8ujvuHk_3Oa2/u/0/?ik=2401283fbe&view=pt&search=all&permthi…1/4 6/6/24, 9:06 AM State of Utah Mail - Fwd: 29107 Pole Patch Compliance https://mail.google.com/mail/b/AEoRXRS0Tq07SjrdoXVFrrL6QBpFhfM9MnHU6JpB8ujvuHk_3Oa2/u/0/?ik=2401283fbe&view=pt&search=all&permthi…2/4 On Tue, Feb 20, 2024 at 8:41 AM Luke Treutel <ltreutel@utah.gov> wrote: Hi Tyson, I have resolved 6 violations worth 90 IPS points since you submitted the chlorine residual data for the Pole Patch system. I am CCing Tom McOmbie, who conducted the 2022 Sanitary Survey of Pole Patch, to see if he can shed some light on the flooding deficiency and provide some feedback on how to resolve it. I followed up with Luke Treutel (MRDL Rule Manager). I also CCed Luke on this reply Colt Smith | Environmental Scientist 385.515.1529 | acsmith@utah.gov | 801.536.4211 (fax) PO Box 144830, 84114-4830 195 North 1950 West, Salt Lake City, UT 84116 Website | Blog | Twitter | Facebook | LinkedIn 6/6/24, 9:06 AM State of Utah Mail - Fwd: 29107 Pole Patch Compliance https://mail.google.com/mail/b/AEoRXRS0Tq07SjrdoXVFrrL6QBpFhfM9MnHU6JpB8ujvuHk_3Oa2/u/0/?ik=2401283fbe&view=pt&search=all&permthi…3/4 On Mon, Feb 19, 2024 at 5:31 PM T J <tysonjack005@gmail.com> wrote: Brandi and Colt, We have reached out to several electrical companies for bids on adding the transfer switch and should have some dates for install for you later this week for the correction on the Significant Deficiency. Below are a few of the other IPS compliance items I thought I had better get something going on since the incorporation into the Pleasant View City Water System is taking so long. Chlorine monitoring for the 3rd and 4th Quarter were submitted. 2023 Lead Consumer Notice I don't believe I remembered to bring them the results so I printed them out and will deliver them tomorrow and will submit the online certification. 2020 Lead Consumer Notice I don't see the results from Chem Tech in my email to do a delayed delivery. How would you like me to proceed with this one? PF001 Pump Station PS17 PS- Piping or Apparatus Leaking. This item was fixed on June 6th 2023. Please let me know if you want pictures to remove it. I don't have any or I would have included it. PS13 - Pump Station floor not protected from flooding or less than 6in above finish grade. No one has explained what they want from us on this one. The building's floor elevation is 6in above the surrounding terrain so water would be forced around the building if any water was ever present as it has still never flooded. Is it just because the pumps are below floor grade and it won't be removed until the pumps themselves are raised?
