PERMIT DEVIATION REPORT
TO: Executive Secretary, Utah Air Quality Board

In accordance with Operating Permit provisions 1.S.2.c, and Utah Administrative Code (UAC)
R307- 415-5a(3XcXii), the following deviation report is submitted:
1. List the dates the deviatlon occurred:Start: Ll3l2O24 Endz41412024
2. Permit conditlon for which the deviation occurred:
Conditions 11.8.1.b.1 .a.iii. Sample shall be analyzed no less than seven days after
co!lection.
3. bescribe the deviatioir that occurred:
Danish Flats Waste Solutions collected the required water sample each month and
requested rushed shipping to the lab for analysis. The lab has been unable to get the
samples analyzed within the seven-day timeframe.
4. Describe the probable cause of the deviation:
Danish Flats is located several hours from any lab that can do the required analysis.
Even when rush shipping is requested, the samples often do not make it to the lab for a
few days. The lab has been subcontracting the methanol sample, which takes several
days to complete the analysis. The lab has been following EPA methods in regard to
analysis times, but the permit-listedT day analysis time is shorter than EPA methods
require.
5. Describe the corrective actlons ot preventatlve measures taken:
Danish Flats Waste Solutions is working with the lab to resolve this issue, but the lab will
likely be unable to meet the required turnaround time for Methanol. Also, Danish Flats
would like to request that the analysis time listed in the permit be extended to match EPA
Method requirements.
