Describe the deviation that occurred:
RTO temperature fel! below 1,360 degrees F for approximately 4 hrs.
Production activities were halted, and lines connected to the RTO remained
down until repairs were completed.
Was the deviation due to and "upset condition" as defined in Section ll of the permit.

Describe the probable cause of the deviation:
The combustion fan motor went out and locked up.
Describe any corrective actions or prevention measures taken:
The motor was replaced with a new motor.
