Operating temperature shall be no less than 1,360 degrees F. [Authority granted under R307-401-8(fXal [BACT]; condition originated in DAQE-ANoLL767OOO8L2I Describe the deviation that occurred: RTo temperature fell below 1,360 degrees F for approximately 19 mins. Was the deviation due to and "upset condition" as defined in Section ll of the permit. NO 5 Describe the probable cause of the deviation: The cool down damper failed to open causing the flame to go out. 6 Describe any corrective actions or prevention measures taken: Filled air line reservoir, cleaned debris, and started back up. ln accordance with Operating Permit Provision l.K and UAC R3O7-415-5b and based on information and belief formed after responsible inquiry, I certify that the statement and information in this document are true, accurate and complete.
