(Terracon) complete the enclosed Work Assignment, under Contract #216383. The Work Assignment is to produce a work plan and price to obtain four title searches for properties of interest to the DERR, and provide a Title Report for each property. Atkinson, PST Branch Manager Division of Environmental Response and Remediation MPA/MEC/rr Enclosure: Work Assignment Title Search-1-TER cc: Thomas Kingston, Division of Environmental Response and Remediation 0-trr= WORK ASSIGNMENT TITLE SEARCH-1-TER Contract #: 216383 Work Assignment: Preparation of a Work Plan and Price for Multiple Property Title Searches The Consultant, Terracon, is requested to complete the following Work Assignment. I. WORK OBJECTIVE: The Division of Environmental Response and Remediation (DERR) has a program for listing and ranking petroleum Release Sites and will, on occasion, obtain a title search of a given property. The objective of the work is to provide a work plan and price to obtain four title searches on properties of interest to the DERR, and providing a Title Report for each property. The work plan and price should provide an estimate of costs for the implementation of the work plan, itemized per task. Each stage of the work plan should be identified as a specific task or sub-task. Work performed under each task or subtask shall not commence without prior approval from the DERR. II. TASKS: The DERR has identified the following tasks: A. Conduct title searches (from the earliest date reasonable to present day) for each of the following properties: i. 360 North Main Street, Logan, Utah Cache County Parcel # 06-043-0014 DERR Release Site: NZR ii. 805 North Redwood Road, North Salt Lake City, Utah Davis County Parcel ID: # 060820270 DERR Release Site: OEH iii. 215 East Main Street, Castle Dale, Utah Emery County Parcel ID: # 1-14-20 DERR Release Site: LGX iv. 2185 South Wall Avenue, Ogden, Utah Weber County Parcel ID: # 030060007 DERR Release Site: KEO B. Provide a Title Report to the DERR for each facility. The Title Search Report shall be prepared by a Licensed Abstractor (as defined the Utah Code)/Title Insurance Agent qualified to search and examine real estate records and titles in the State of Utah. The report shall contain legible copies of the tiles, deeds, Lease agreements, and other available documents of ownership. The title report shall also include a summary table of the property ownership history that lists each transaction chronologically and identifies the type of transaction, recording date, and the parties involved. III. PRICE AND PAYMENT SCHEDULE: A. The expense to the DERR for completing this Work Assignment shall not exceed $1,100.00, including administrative costs, contract management, project management, and other associated costs. B. Payment requests shall be submitted when tasks are completed, as outlined in the attached Deliverable and Payment Schedule. IV. SCHEDULE FOR COMPLETION OF WORK AND REPORTING REQUIREMENTS: A. The DERR has set a deadline of May 2024, for Terracon to submit a draft work plan and price for the determined work. B. After the DERR has reviewed and commented on the draft work plan and price, Terracon will be required to provide a final work plan and price within 4 days of receipt of any DERR comments.
