Dear Mr. Crim:

Kleinfelder completed the scope of work consisting of locating and exposing one monitoring
well and two vent wells included in work assignment GQJ-88-KLE on May 1 and 2, 2024.
Kleinfelder’s field staff met with Terry and Wayne Wiseman and Paul and Melissa Wunderlich at
Scotty’s Diner, in Milford, Utah. Upon arrival, Kleinfelder began searching for the three
monitoring and soil-vent wells. After locating each well, Kleinfelder assessed the condition of
the well completion, the well casing, and the well backfill (to the extent possible). If the well
completion was covered with gravel or other fill material, Kleinfelder used a metal detector to
search for the metal well vault. Each located well was gauged with an oil/water interface probe.
All wells assessed as part of this scope of work were dry or only contained residual water in the
bottom of the casing.

Monitoring well MW-27 and vent well VW-16 were found north of the Site, behind a historical
building that is currently being renovated. MW-27 was found approximately two inches
below the surface of some fill material and VW-16 was visibly sticking out of the ground
(Attachment 1). VW-13 was found in the northeast corner of the Scotty’s Diner property under
approximately three inches of gravel (Attachment 1).

A summary of the well inspection and assessment activities is included in Table 1, attached to
this letter. Additionally, the Site Map (Figure 1) has been updated to show the actual locations
of the three wells that were located during this investigation.
