Remediation (DERR) project manager who has recommended that no further corrective action be
taken at this time. This no further action applies only to the specific Release Site FOP. This
recommendation is based on the information contained in the file for this release. That information
was supplied by you, your consultant, or another representative.
The information submitted indicates that any detectable petroleum contamination from this
release is not a threat to human health or the environment according to Utah Admin. Code rule
R311-211. In the future, if other evidence indicates a spread of contamination from this release
which may cause a threat, further corrective action may be required.

SUBJECT: APST NFA - Closure Review Memo
St. Mark s Hospital, located at 1200 East 3900 South, Salt Lake City, Utah
Facility Identification No. 4001116, Release Site OIO

Minor line leaks were discovered in late July 2023 during system testing for a new diesel APST
servicing the Plant Facility at the hospital Rainwater inundated the line trench where leaked product
was being collected in a tray while repairs were on-going. The product, rainwater and impacted soils
were removed from the trench. The final confirmation soil sample was below detection levels or
below ISLs for all compounds.
