Dear Brad Stapley:
You are required to submit an exception request to the Division of Drinking Water (the Division) along with confirmation that all conditions outlined in this temporary exception have
been fulfilled and obtain an exception renewal prior to (Expiration Date). Requirements for Rule Exceptions can be found at https://deq.utah.gov/drinking-water/exceptions-to-rule-requirements.
After receiving your request, the Division will determine whether this exception can be re-issued.
Exceptions granted by the Division include an expiration date to ensure that the basis and conditions for granting the exception are verified periodically and that the drinking water
quality remains protected.
Failure to submit the exception request and obtain a renewal prior to the current expiration date will result in the assessing of any deficiencies related to the Rule.
