OPERATING STATUS: Operating normally.
PROCESS DESCRIPTION: Salt Lake City Corporation operates three boilers and two
generators for the Salt Lake City and County Building, the
Leonardo Museum, and the Salt Lake City Public Library. The
9.8 MMBtu/hr boilers operate solely on natural gas, while the
6.0 MMBtu/hr boiler can operate on natural gas or diesel fuel.
Diesel fuel is only used during times of curtailment or testing.
The two emergency generators provide emergency power to the
complex. The boilers and one generator are located on the south
side of the underground parking structure for the library, with
entry to the boiler room on the lower level.

Section I: GENERAL PROVISIONS
I.1 All definitions, terms, abbreviations, and references used in this AO conform to those used in the
UAC R307 and 40 CFR. Unless noted otherwise, references cited in these AO conditions refer to
those rules. [R307-101]
I.2 The limits set forth in this AO shall not be exceeded without prior approval. [R307-401]
I.3 Modifications to the equipment or processes approved by this AO that could affect the emissions
covered by this AO must be reviewed and approved. [R307-401-1]
I.4 All records referenced in this AO or in other applicable rules, which are required to be kept by
the owner/operator, shall be made available to the Director or Director's representative upon
request, and the records shall include the two-year period prior to the date of the request. Unless
otherwise specified in this AO or in other applicable state and federal rules, records shall be kept
for a minimum of two (2) years. [R307-401-8]
I.5 At all times, including periods of startup, shutdown, and malfunction, owners and operators shall,
to the extent practicable, maintain and operate any equipment approved under this AO, including
associated air pollution control equipment, in a manner consistent with good air pollution control
practice for minimizing emissions. Determination of whether acceptable operating and
maintenance procedures are being used will be based on information available to the Director
which may include, but is not limited to, monitoring results, opacity observations, review of
operating and maintenance procedures, and inspection of the source. All maintenance performed
on equipment authorized by this AO shall be recorded. [R307-401-4]
I.6 The owner/operator shall comply with UAC R307-107. General Requirements: Breakdowns.
[R307-107]
I.7 The owner/operator shall comply with UAC R307-150 Series. Emission Inventories. [R307-150]
I.8 The owner/operator shall submit documentation of the status of construction or modification to
the Director within 18 months from the date of this AO. This AO may become invalid if
construction is not commenced within 18 months from the date of this AO or if construction is
discontinued for 18 months or more. To ensure proper credit when notifying the Director, send
the documentation to the Director, attn.: NSR Section. [R307-401-18]
Status: In Compliance. No limits appear to have been exceeded. No modifications have been
made to equipment or process. Records were provided by email after the inspection. No
reportable breakdowns have occurred since the previous inspection. The source submitted
a 2020 emission inventory, however a new AO was issued in 2022 which changed the
potential to emit values. All equipment was installed at the time the AO was issued.

Status: In Compliance. The equipment observed on site was consistent with that listed.
The source is planning to replace the generators with newer and larger models,
and a new AO, DAQE-AN128150003-24, dated April 29, 2024, was issued to
make the generator change.
II.B Requirements and Limitations
II.B.1 Limitations
II.B.1.a The owner/operator shall use natural gas as a primary fuel and #1 or #2 grade fuel oil (diesel
fuel) as a backup fuel in the 150 HP boiler. [R307-401-8]
Status: In Compliance. Only natural gas has been used in the boilers in the last 12 months.
No diesel fuel has been used in the 150 HP boiler.
II.B.1.b The owner/operator shall not operate the 150 HP boiler for more than 48 hours per rolling 12-
month total during periodic testing, maintenance, or operator training on liquid fuel. There is no
time limit on the use of the 150 HP boiler during gas curtailment, gas supply interruption, or
startups. [R307-401-8]

II.B.1.b.1 The owner/operator shall:
A. Determine hours of operation by monitoring and maintaining an operations log
B. Record hours of operation and reason for operation on liquid fuel each day
C. Use the hours of operation to calculate a new rolling 12-month total by the 20th day of
each month using data from the previous 12 months
D. Keep hours of operation records for all periods the plant is in operation. [R307-401-8]
Status: In Compliance. No diesel fuel has been used in the 150 HP boiler in the last 12
months.
II.B.1.c The owner/operator shall not consume more than the following:
A. 25,350 gallons of diesel fuel (#1 or #2 grade fuel oil) per rolling 12-month period
B. 166,486,100 standard cubic feet of natural gas per rolling 12-month period.

II.B.1.c.1 The owner/operator shall:
A. Determine consumption by examination of the individual fuel supplier billing records
B. Record consumption of natural gas and diesel fuel on a daily basis
C. Use the records of natural gas and diesel fuel consumption to calculate a new rolling
12-month total by the 20th day of each month using data from the previous 12 months
D. Keep the consumption records for all periods the office/library complex is in operation.

Status: In Compliance. In the rolling 12-month period, 147 gallons of diesel fuel was used
and 17,450,997 scf of natural gas. See attached.
II.B.1.d Visible emissions from the following emission points shall not exceed the following values:
A. All boilers fueled by natural gas - 10% opacity
B. All boilers fueled by diesel fuel - 20% opacity
C. All diesel engines - 20% opacity
D. All other points - 20% opacity. [R307-401-8]
II.B.1.d.1 Opacity observations of emissions from stationary sources shall be conducted in accordance with
40 CFR 60, Method 9. [R307-201-3]
Status: In Compliance. No visible emissions were observed from any point during the
inspection.
II.B.2 Emergency Engines
II.B.2.a The owner/operator shall not operate each emergency engine on site for more than 100 hours per
rolling 12-month period during non-emergency situations. There is no time limit on the use of
the engines during emergencies. [R307-401-8, 40 CFR 63 Subpart ZZZZ]

II.B.2.a.1 To determine compliance with a rolling 12-month total, the owner/operator shall calculate a new
12-month total by the 20th day of each month using data from the previous 12 months. Records
documenting the operation of each emergency engine shall be kept in a log and shall include the
following:
A. The date the emergency engine was used
B. The duration of operation in hours
C. The reason for the emergency engine usage
D. Keep the consumption records for all periods the office/library complex is in operation.

II.B.2.a.2 To determine the duration of operation, the owner/operator shall install a non-resettable hour
meter for each emergency engine. [R307-401-8, 40 CFR 63 Subpart ZZZZ]
Status: In Compliance. The 12-month rolling total for generator operation was 25.2 hours
for the 500 kW engine and one hour for the 278 kW engine. Fuel consumed during this
period was 147 gallons. Each generator is equipped with a non-resettable hour meter, the
500 kW's read 379.6 hours, and the 278 kW's read 286.1 at the time of inspection. See
attached.
II.B.2.b The owner/operator shall only use diesel fuel (e.g. fuel oil #1, #2, or diesel fuel oil additives) as
fuel in each emergency engine. [R307-401-8]
II.B.2.b.1 The owner/operator shall only combust diesel fuel that meets the definition of ultra-low sulfur
diesel (ULSD), which has a sulfur content of 15 ppm or less. [40 CFR 63 Subpart ZZZZ,
R307-401-8]
II.B.2.b.2 To demonstrate compliance with the ULSD fuel requirement, the owner/operator shall maintain
records of diesel fuel purchase invoices or obtain certification of sulfur content from the diesel
fuel supplier. The diesel fuel purchase invoices shall indicate that the diesel fuel meets the ULSD
requirements. [R307-401-8]
Status: In Compliance. Diesel fuel is purchased through Sinclair Oil and is certified to be
15 ppm or less sulfur content. See attached.
Section III: APPLICABLE FEDERAL REQUIREMENTS
In addition to the requirements of this AO, all applicable provisions of the following federal programs
have been found to apply to this installation. This AO in no way releases the owner or operator from any
liability for compliance with all other applicable federal, state, and local regulations including UAC
R307.
MACT (Part 63) JJJJJJ: National Emission Standards for Hazardous Air Pollutants for Industrial, Commercial,
and Institutional Boilers Area Sources
Status: In Compliance. The 150 HP boiler has not operated on diesel fuel in the last 12 months.
MACT (Part 63) ZZZZ: National Emissions Standards for Hazardous Air Pollutants for Stationary
Reciprocating Internal Combustion Engines
Status: In Compliance. The generator engines have operated for less than 100 hours in the last 12
months for testing and maintenance. Each engine is equipped with a non-resettable hour meter. Regular
maintenance is conducted, the last oil changes and annual maintenance check took place on September
23, 2023. An operation log is kept with each engine.

AREA SOURCE RULES EVALUATION:
The following Area Source Rules were evaluated during this inspection:
R307-214. National Emission Standards for Hazardous Air Pollutants
Status: In Compliance. This condition is met by compliance with ZZZZ. The 150 HP boiler did not
operate on diesel fuel in the last 12 months.
R307-309. Nonattainment and Maintenance Areas for PM10 and PM2.5: Fugitive Emissions and Fugitive
Dust
Status: In Compliance. No visible emissions were observed during the inspection. The grounds are
either paved or landscaped.

ACTIONS: No enforcement actions within the past five years.
COMPLIANCE STATUS &
RECOMMENDATIONS: In regards to Approval Order (AO) DAQE-AN128150002-22,
dated March 3, 2022: the source should be considered in
compliance. The facility appeared to be well maintained and
operated properly. Records were provided by email post
inspection.
HPV STATUS: Not Applicable.

RECOMMENDATION FOR
NEXT INSPECTION: Inspect as normal. Look for the new AO
DAQE-AN128150003-24, dated April 29, 2024. The source is
planning to replace the emergency generators. The inspector may
also want to contact Jeff Katter prior to the visit, his office is off
campus a few blocks away at 248 East 600 South, and is a
secured location.
NSR RECOMMENDATIONS: None at this time.

PMI - ; PMI - Traveled to location, completed inspection on the unit for service needs. Inspected the engine oil, the engine coolant, the engine
drive belts, the operating condition of the engine, and the engine components. Found several items that need to be addressed, parts request
handed to Kathy. See other lines for repairs needed. Completed. ; Repairs needed - The unit needs a new block heater, a new oil temp sensor,
and a new coolant level sensor. The decision has been made to forgo this portion of the repair as the unit is slated to be replaced in the mid
future as per Dru. The safety shut down on the engine is still operative, this is only for the gauges on the panel. ; ALEX WILL GET THE MODEL
NUMBER SO WE CAN ORDER PARTS. - ; UPDATE NO PARTS ORDERED WAITING FOR ALEX TO GET INFO. - ; PO-Engine Heater from Napa by end
of day. - ; Block heater - Traveled to the City County building and met with the Electricians for Facilities and talked to them about the needs of
the unit for repair and retrofit to plugs for the new block heater that will be installed on the unit. After the plugs were installed, retuned and
installed a 2000 watt heater to see if this can keep the engine warmer and allow for better runability during exercise. Installed new fitting in the
engine that were obtained from Hose and Rubber including shut off valves to allow for future repair as needed. Installed new hoses for the heater
as well. Completed. ; PR-Heater - ; PO- Muffler Sealer from Parts Authority ETA 1/31/24 - ; Exhaust repair - Traveled to location, found that the
gasket between the turbo and the exhaust piping has failed fully and is in need of replacement. Unable to locate a suitable replacement due to
the age on the genset. Ordered Permatex Ceramic exhaust seal and removed the pipe section that was leaking. Cleaned with wire wheel and
installed back on the unit with the ceramic sealer. Found that there is oil coming from the pipe, possible the turbo outlet. Will check at next PM
to see if the problem needs to be addressed. Confirmed no further leaks. Completed. ; Reviewed & Closed -

PMB - ; PMI - ; GETTING ALARM FOR LOW ENGINE TEMP...HAS BEEN ONGOING ISSUE - FOR SEVERAL WEEKS THIS ALARM HAS BEEN GOING
OFF...THEY CALL A FEW TIMES A WEEK TO REPORT SEAN ODELL 801-347-7472 ; Engine warning - Traveled to the library and checked the unit
for issues. Found that the block heater is not operating normally. Noted that the block heater was recently replaced. Went to the breaker room
and found that the breaker for the heater circuit was tripped. Reset breaker and found that the block heater is still not functioning normally.
Found that the 210V relay that is on the unit for the block heater is currently stuck in the open position with some minor scorch marks on the
contacts. Traveled to Fleet and ordered the block heater relay, with a block heater just in case both are needed. Will attempt to replace the relay
first then the block heater as needed after that. ; PR- Block Heater - Received & at the parts counter. ; PR- Block Heater Relay - Received & at
the parts counter. ; PO- Oil Filter ETA 6/12/23 - Ordered from Napa. ; PR- Oil Filters - Received & at the parts counter. ; Block heater - Traveled
to location and installed the previously ordered relay for the block heater. After powering the system up, found that the block heater is working
as designed and is now operational. Reset the screen warnings and confirmed that the unit is working normally. Completed. ; PMB - Traveled to
location and completed replacement of the engine coolant filter, engine air filter, the two engine oil filters, and the two engine fuel filters.
Completed. ; PMF - Completed engine oil change with the replacement of all the filters. ; PMI - Completed inspection of the unit for service
needs. Inspected the engine assembly, the engine cooling system, the drive belt, and the overall condition of the engine. Ran the generator for
30 minutes after service was complete and found no issues. Completed. ; Reviewed & Closed -

PMI - ; PMI - Traveled to location, found that the block heater was not functioning. Found that the circuit breaker for the unit was tripped. Reset
the breaker and spoke to facilities. Told them that the new unit should not be the issue and to have the electricians come check the circuit. After
finding that, the engine would not start due to a low coolant condition. Returned later and found that the electricians were installed new wiring
for the heater circuit and that there was a new breaker that is a 30 amp breaker being installed. After confirming that the engine was OK to run,
ran and inspected for service needs. See lines for repairs PMI Completed. ; Exhaust leak - Found that there are two exhaust leaks on the unit.
One from the turbo to manifold flange and the other from the split in the exhaust manifold at the rear of the engine. P/A from parts then repair
will be decided on by Dru. ; Repair another time - After speaking with Dru, the exhaust repair will completed when the issue gets worse.
Currently not a concern due to the unit being outside and well vented. ; reviewed and closed -
