RE: Request for Operating Permit for Bryce Canyon Shadows RV Park

Dear Paul Wright:

Regarding the above referenced project please note the following:

1. The project was completed according to the approved drawings and specifications, with
minor changes that do not affect water quality or quantity and are in compliance with Utah
Administrative Rules.
2. One set of construction record drawings has been given to the Owner.
3. Flushing and disinfection of the new water lines has been completed in accordance with
appropriate AWWA/ANSIS standards.
4. Bacteriological samples have been taken for the well and water line and are included with
this letter.
5. An Operation and Maintenance manual has been sent to the owner.
