Box 37 Corinne, Utah 84307 Subject:Permanent Operating Permit, Main (Baker) Spring (WS004) Redevelopment; West Corinne Water Company, System #02022, File #11284, SRF #3F305 Dear Cary McFarland:The Division of Drinking Water (the Division) received your request for a Permanent Operating Permit for the Main (Baker) Spring on The Division issued Plan Approval for this project on April 3, 2020. The Division issued a temporary Operating Permit for this project on November 13, 2020. Our understanding of the project was to redevelop the Main (Baker) Spring (WS004), construct two (2) new spring collection boxes, one (1) junction box, approximately 3,300 feet of 8-inch diameter HDPE water line, five (5) air relief vents, one (1) pressure reducing valve vault and one (1) pressure sustaining valve vault. The Main (Baker) Spring is identified as WS004 in the Division’s database. We have received the following information for Main (Baker) Spring: Design engineer’s statement of conformance with approval conditionsand that changes made during construction were in conformance with rules R309-500 through 550. Changes included: The original design was for three (3) collection boxes to be piped to one (1) junction box. However, during construction, it was decided to combine two (2) collection boxes into one (1) larger box leaving two (2) collection boxes, total, piped to the junction box. An impermeable liner was added under the collection area. An additional side spring was discovered and connected to an 8-inch diameter HDPE line. A new line from the pressure relief vault was extended to create a new location for the air gap rather than being connected to the existing pipe air gap. A different reducer size was used to connect the pipe from the pressure sustaining vault to the existing pipe entering the tank. The need for extra fittings and pipe to connect to the side springs. Three years of monthly spring flow data. Satisfactory bacteriological results as evidence of proper disinfection and flushing. Water quality data — satisfactory updated nitrate results. Operating Permit for Main (Baker) Spring (WS004) We have determined that all conditions of issuing an Operating Permit have been accomplished. On this basis, an Operating Permit for Main (Baker) Spring is hereby issued as constituted by this letter. You may retain Main (Baker) Spring (WS004) in service in your water system. Please maintain a copy of this letter with your permanent records for future reference.The yield of Main (Baker) Spring (WS004) is determined to be 173 gallons per minute (gpm) based on the spring flow information we received. This yield is the basis for determining the maximum number of connections that the Main (Baker) Spring can serve. Bacteriological Source Sampling This system is required to take monthly Escherichia Coli (E. coli) sampling at Main (Baker) Spring (WS004) (assessment monitoring) during the operating period when the spring is accessible and shall continue for a minimum of 6 months. This schedule will begin June 2024. This sample is in addition to the monthly distribution system sample (DS001). Please label the source sample as WS004 (for both the facility ID and sample point ID) on all laboratory forms. Please contact Sitara Federico for any positive samples that may occur at the source at 385-515-1459 or sfederico@utah.gov. If any sample result comes back as E. coli positive, please contact the Division within 24 hours of being notified of the result. This monitoring is required to evaluate the quality of the water coming from Main (Baker) Spring to better protect public health. Additional monitoring and/or disinfection requirements may be assigned depending on the results of the assessment monitoring.The issuance of this Operating Permit resolves your water system’s S001 deficiency related to an UNAPPROVED SOURCE IN SERVICE.
