Subject:Operating Permit, Water Line Replacement, Bench Creek Road PRV Vault, and High Tank (ST003); Woodland Mutual Water Company, System #22018, File #11997
Dear Andy Crnich:The Division of Drinking Water (the Division) received a request for an Operating Permit for system wide improvements from your consultant, Cliff Linford, P.E. with
Sunrise Engineering on May 10, 2024. The Division issued Plan Approval for this project on March 19, 2020.
Our understanding of the project includes the following:
325,000-gallon buried concrete storage tank referred to as High Tank (identified as ST003 in the Division’s database)
Bench Creek Road PRV Vault
Replacement of approximately 5 miles of PVC water line through the system (DS001)
Improvements to the Woodland Spring (WS001), Cemetery Booster Pump and 300 feet of water line along Highway 35 North were included in the plan approval but were removed from the project.
The Woodland Spring (WS001) flow has seen a significant reduction in recent years. Due to the lack of water production from the spring, Woodland Mutual Water Company elected to remove
the improvements and the booster vault from the project which was designed to boost water to the High Tank (ST003). The proposed water line along Highway 35 was removed from the project
due to the low number of houses that would have been served by the line, which are now being served by Francis Town.
We have received the following information for the water line replacement, PRV vault, and High Tank (ST003):
Certification of Rule Conformance with plan approval conditions by a professional engineer As-built or record drawings.
Satisfactory bacteriological results as evidence of proper disinfection and flushing.
Evidence of O&M manual delivery.
We have determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for the Water Line Replacement, PRV vault and High Tank (ST003)projectis
hereby issued as constituted by this letter. You may continue to use these facilities in service in your water system.
Please maintain a copy of this letter with your permanent records for future reference.The issuance of this Operating Permit resolves your water system’s deficiency related to having
an unapproved facility in service (G001). An updated IPS report isavailable at https://waterlink.utah.gov. Please contact Jennifer Yee at (385) 515-1501 or jyee@utah.gov for questions
