2. Permit conditions for which deviation occurred:
1.B.3.a Operating temperature shall be no less than 1,360 degrees F. [Authority granted under
R3O7-401-8(r) (a) [BACT]; condition originated in DAQE-AN0117670008-12I
3. Describe the deviation that occurred:
RTO temperature fell betow 1,360 degrees F for approximately 15 minutes the first occurrence
and 2 hours and 21 minutes the second occurrence.
4. Was the deviation due to an "upset condition" as defined in Section ll of the permit?

5. Describe the probable cause of deviation:
A defective mac air valve and a failing relay shut unit down.
6. Describe any corrective actions or preventive measures taken:
Replaced bad mac valve and ice cube relay.
