(3XcXii), the following deviation report is submitted.
1 Dates that the deviation occurred:

2 Permit conditions for which deviation occurred:
ll.B.3.a. Operating temperature shall be no less than 1,360 degrees F. [Authority granted
under R307-401-8(fXa) [BACT]; condition originated in DAQE-AN011767000812I
Describe the deviation that occurred:
RTO temperature fell below 1,360 degrees F for approximately 15 hrs. Production activities
were halted, and lines connected to the RTO remained down untilthe repairs were
completed.
Was the deviation due to and "upset condition" as defined in Section ll of the permit.

5 Describe the probable cause of the deviation:
The north tower main shaft wore through and broke in 3 places.
6 Describe any corrective actions or prevention measures taken:
The shaft was replaced, cool down damper switch also replaced.
