Dear Mr. Hansen.
In accordance with condition 1.Q.1 of Clean Harbors Aragonite's (CHA) State RCRA Permit,
this correspondence serves as written notification that the ref'erenced facility did not incinerate
two (2) infectious waste containers within 60 days, which is a violation of Condition 3.D.23.
On March 4,2024, it was discovered that infectious w'aste container A00CKC0I had not been
incinerated within 60 days and was still in storage. This container arrived at the facility on
December 16,2023. and has not yet been incinerated (60 days in exceedance). This issue was
caused by originally finalizing the containers as missing on the manifest. When the container
later showed up at plant. the container was still considered unresolved and was placed on the
refrigerated trailer.
On March 5,2024. it was discovered that infectious waste container A0OCVMAR had not been
incinerated within 60 days and was still in storage. This container arrived at the facility on
December 8, 2023, and has not yet been incinerated (60 Days in exceedance). This issue was
caused by improper tracking ofthe container after it received scanning.
To correct these issues, CHA has retrained the operators involved and have corrected the
infectious waste report. In addition, the facility will verifr all containers that are inside of each
refrigerated trailer. on a monthly basis by a physical removal and verifo of each container. These
containers will be incinerated once the facility is back online from the scheduled outage.
