DIRECTIONS: Follow signs for Clean Harbors Clive Facility once you exit
I-80. The office is located in the modular trailer on the south side
of the security fence. Ring the doorbell by the security gate or
call the source contact number on the gate to gain access.

OPERATING STATUS: Operating normally at the time of inspection.
PROCESS DESCRIPTION: This source is currently used for transfer and storage of materials
only. Mixing or treatment does not occur at this site, but
adsorbents may be applied during loading processes. The
incinerator has not operated for years and is now partially
dismantled and unable to operate. Materials to be incinerated are
taken to the Aragonite or Grassy Mountain facilities. Some
incoming wastes are screened to determine acceptability.
Wastes are brought in by rail, trucks, and tankers, and include
non-energetic and energetic liquids, non-energetic and energetic
solids and soils, containerized solids, and other wastes such as
medical wastes. Roll boxes containing wastes are stored in one
of the RCRA permitted storage areas.

Section I: GENERAL PROVISIONS
I.1 All definitions, terms, abbreviations, and references used in this AO conform to those used in the
UAC R307 and 40 CFR. Unless noted otherwise, references cited in these AO conditions refer to
those rules. [R307-101]
I.2 The limits set forth in this AO shall not be exceeded without prior approval. [R307-401]
I.3 Modifications to the equipment or processes approved by this AO that could affect the emissions
covered by this AO must be reviewed and approved. [R307-401-1]
I.4 All records referenced in this AO or in other applicable rules, which are required to be kept by
the owner/operator, shall be made available to the Director or Director's representative upon
request, and the records shall include the two-year period prior to the date of the request. Unless
otherwise specified in this AO or in other applicable state and federal rules, records shall be kept
for a minimum of two (2) years. [R307-401-8]
I.5 At all times, including periods of startup, shutdown, and malfunction, owners and operators shall,
to the extent practicable, maintain and operate any equipment approved under this AO, including
associated air pollution control equipment, in a manner consistent with good air pollution control
practice for minimizing emissions. Determination of whether acceptable operating and
maintenance procedures are being used will be based on information available to the Director
which may include, but is not limited to, monitoring results, opacity observations, review of
operating and maintenance procedures, and inspection of the source. All maintenance performed
on equipment authorized by this AO shall be recorded. [R307-401-4]
I.6 The owner/operator shall comply with UAC R307-107. General Requirements: Breakdowns.
[R307-107]
I.7 The owner/operator shall comply with UAC R307-150 Series. Emission Inventories. [R307-150]
Status: In Compliance. Each condition from Section I was reviewed and determined to be
in compliance. The source is not currently required to submit an Emission Inventory.
Section II: SPECIAL PROVISIONS
II.A The approved installations shall consist of the following equipment:

Status: In Compliance. No unpermitted equipment was observed during the
inspection.
Additional Information:
II.A.3: The bulb recycling unit was unable to be viewed as it was operating and
requires a respirator for entry.
II.A.4: The diesel fuel fire pump engine was replaced with a fully electric
engine in January 2023.
II.A.7: One 500-gallon storage tank was removed in concurrence with the
diesel engine replacement.
II.B Requirements and Limitations
II.B.1 Source-wide Requirements
II.B.1.a The owner/operator shall not transfer more than 13,000,000 gallons of liquid waste per rolling
12-month period. [R307-401-8]
II.B.1.a.1 The owner/operator shall:
A. Determine amount of liquid waste transferred with customer receipts or billing
records
B. Record amount of liquid waste transferred on a daily basis
C. Use the transfer data to calculate a new rolling 12-month total by the 20th day
of each month using data from the previous 12 months. [R307-401-8]
Status: In Compliance. The source transferred 202,471 gallons of liquid waste for the
rolling 12-month period ending in October 2023. Values are calculated according to this
condition. Refer to the attachments for more details.

II.B.1.b The owner/operator shall use a carbon adsorption system to control emissions from the transfer
of liquid waste from rail cars to trucks. [R307-401-8]
Status: In Compliance. A carbon adsorption system is used and was viewed during the
inspection.
II.B.1.c The owner/operator shall maintain and operate the carbon adsorption system according to the
manufacturer's instructions or recommendations. [R307-401-8]
Status: In Compliance. The carbon adsorption system is maintained and operated
according to the manufacturer's recommendations.
II.B.1.d The owner/operator shall transfer liquid waste on site by the use of submerged loading.
[R307-401-8]
Status: In Compliance. Submerged loading is utilized to transfer liquid waste.
II.B.1.e Unless otherwise specified in this AO, the owner/operator shall not allow visible emissions from
any stationary point or fugitive emission source on site to exceed 10% opacity. [R307-401-8]
II.B.1.e.1 Unless otherwise specified in this AO, opacity observations of emissions shall be conducted
according to 40 CFR 60, Appendix A, Method 9. [R307-201-3]
Status: In Compliance. No visible emissions were observed from any stationary point or
fugitive source. Refer to the VEO Form in the attachments.
II.B.2 Bulb Recycling System Requirements
II.B.2.a The owner/operator shall use two baghouses in parallel and an activated carbon filter system to
control emissions from the Lamp Recycler. [R307-401-8]
Status: In Compliance. The source indicated that two baghouses are used in parallel along
with an activated carbon filter system. The baghouse gauges could not be safely viewed as
the Bulb Recycling Unit was operating and requires a respirator for entry. Records
provided by the onsite personnel indicate the process is closely monitored and procedures
are followed according to this AO.
II.B.2.b The owner/operator shall use a baghouse and an activated carbon filter to control emissions from
the Shatter Shield Processor. [R307-401-8]
Status: In Compliance. The source indicated that a baghouse and an activated carbon filter
system are used to control emissions from the Shatter Shield Processor. The baghouse
gauges could not be safely viewed as the Bulb Recycling Unit was operating and requires a
respirator for entry. Records provided by the onsite personnel indicate the process is
closely monitored and procedures are followed according to this AO.
II.B.2.c The owner/operator shall maintain and replace each activated carbon filter according to the
manufacturer's instructions or recommendations. [R307-401-8]
Status: In Compliance. The source indicated that each carbon filter is maintained and
replaced according to manufacturer recommendations.
II.B.2.d The owner/operator shall install a manometer or magnehelic pressure gauge to measure the static
pressure drop across each baghouse in the bulb recycling system. [R307-401-8]
II.B.2.d.1 The pressure gauge shall be located such that an inspector/operator can safely read the indicator
at any time. [R307-401-8]

II.B.2.d.2 The pressure gauge shall measure the pressure drop in 1-inch water column increments or less.
[R307-401-8]
Status: In Compliance. The source confirmed that a pressure gauge is located on each
baghouse and measurements are recorded daily. The Bulb Recycling Unit was operating
and the gauges could not be safely viewed without a respirator. The onsite personnel
provided records of the previous months' daily pressure drop readings and all values were
within the specified range of 2 and 9 inches of water column.
II.B.2.e The owner/operator shall maintain the static pressure drop across each bulb recycling system
baghouse during operation between 2 and 9 inches of water column. [R307-401-8]
II.B.2.e.1 The owner/operator shall record the pressure drop at least once per operating day. [R307-401-8]
II.B.2.e.2 The owner/operator shall maintain records of the pressure drop readings. [R307-401-8]
Status: In Compliance. The source confirmed that a pressure gauge is located on each
baghouse and measurements are recorded daily. The Bulb Recycling Unit was operating
and the gauges could not be safely viewed without a respirator. The onsite personnel
provided records of the previous months' daily pressure drop readings and all values were
within the specified range of 2 and 9 inches of water column. Refer to the attachments for
the recorded pressure drop values.
II.B.3 Emergency Engine Requirements
II.B.3.a The owner/operator shall install a 234 hp emergency engine that is certified to meet a
NMHC+NOx emission rate of 3.0 g/hp-hr or less. [R307-401-8]
II.B.3.a.1 To demonstrate compliance with the emission rate, the owner/operator shall keep a record of the
manufacturer's certification of the emission rate. The record shall be kept for the life of the
equipment. [R307-401-8]
Status: Not Applicable. The 234 hp emergency engine (II.A.4) was replaced with an
all-electric unit and no longer requires an emission rate certification.
II.B.3.b The owner/operator shall not operate each emergency engine on site for more than 100 hours per
rolling 12-month period during non-emergency situations. There is no time limit on the use of
the engines during emergencies. [R307-401-8, 40 CFR 63 Subpart ZZZZ]
II.B.3.b.1 To determine compliance with a rolling 12-month total, the owner/operator shall calculate a new
12-month total by the 20th day of each month using data from the previous 12 months. Records
documenting the operation of each emergency engine shall be kept in a log and shall include the
following:
A. The date the emergency engine was used
B. The duration of operation in hours
C. The reason for the emergency engine usage. [40 CFR 60 Subpart ZZZZ, R307-401-8]
II.B.3.b.2 To determine the duration of operation, the owner/operator shall install a non-resettable hour
meter for each emergency engine. [R307-401-8, 40 CFR 63 Subpart ZZZZ]
Status: In Compliance. A non-resettable hour meter was observed on the emergency diesel
engine (II.A.5). The engine operated for 10.1 hour for non-emergency usage for the rolling
12-month period ending in October 2023. A usage log is maintained digitally and contains
the date, duration, and reason for engine usage. Refer to the usage log in the attachments.

II.B.3.c The owner/operator shall not allow visible emissions from the emergency engines on site to
exceed 20% opacity. [R307-401-8]
Status: In Compliance. The emergency engine was not operating at the time of inspection
and no visible emissions were observed.
II.B.3.d The owner/operator shall only use diesel fuel (e.g. fuel oil #1, #2, or diesel fuel oil additives) as
fuel in each emergency engine. [R307-401-8]
II.B.3.d.1 The owner/operator shall only combust diesel fuel that meets the definition of ultra-low sulfur
diesel (ULSD), which has a sulfur content of 15 ppm or less. [R307-401-8]
II.B.3.d.2 To demonstrate compliance with the ULSD fuel requirement, the owner/operator shall maintain
records of diesel fuel purchase invoices or obtain certification of sulfur content from the diesel
fuel supplier. The diesel fuel purchase invoices shall indicate that the diesel fuel meets the ULSD
requirements. [R307-401-8]
Status: In Compliance. Only ultra-low sulfur diesel is used as fuel for the emergency
engine. A certificate of analysis provided by the fuel supplier indicated a sulfur content of
4.8 ppm. Refer to the attachments for more details.
II.B.4 Storage Tank Requirements
II.B.4.a The owner/operator shall load the 10,000-gallon diesel storage tank by the use of submerged
loading. [R307-401-8]
Status: In Compliance. The 10,000-gallon diesel storage tank is loaded via submerged
loading.
Section III: APPLICABLE FEDERAL REQUIREMENTS
In addition to the requirements of this AO, all applicable provisions of the following federal programs
have been found to apply to this installation. This AO in no way releases the owner or operator from any
liability for compliance with all other applicable federal, state, and local regulations including UAC
R307.
NSPS (Part 60) IIII: Standards of Performance for Stationary Compression Ignition Internal Combustion
Engines
Status: Not Applicable. The emergency engine subject to this subpart has been replaced with an electric
engine and is no longer applicable.
MACT (Part 63) -ZZZZ: National Emissions Standards for Hazardous Air Pollutants for Stationary
Reciprocating Internal Combustion Engines
Status: In Compliance. Refer to Section II.B.3 for more details.
AREA SOURCE RULES EVALUATION:
The following Area Source Rules were evaluated during this inspection:

Status: In Compliance. Refer to Condition II.B.3.d.2 for more details.

Emission Standards: Fugitive Emissions and Fugitive Dust [R307-205]
Status: In Compliance. No fugitive emissions or dust were observed during the inspection. The site is
mostly paved.
National Emission Standards for Hazardous Air Pollutants [R307-214]
Status: In Compliance. This rule is satisfied by compliance with 40 CFR Part 63 Subpart ZZZZ.
Refer to the Federal Requirements Section and Section II.B.3 for more details.
