OPERATING STATUS: Operating.
PROCESS DESCRIPTION: This source prints various items including periodicals on the
Lithoman, Timson, two Man Roland presses, or other smaller
printing devices. Plates pickup and deliver ink to paper. The inks
can be heat set, air dried, or light cured. Emissions from the
Lithoman and Timson presses are passed through a regenerative
thermal oxidizer before being released to the atmosphere through
a stack. Paper and paper dust emissions from the cutting room
are transferred to one of four baghouses by negative pressure
ducts. Baghouses vent onto the roof under the units.

Section I: GENERAL PROVISIONS
I.1 All definitions, terms, abbreviations, and references used in this AO conform to those used in the
UAC R307 and 40 CFR. Unless noted otherwise, references cited in these AO conditions refer to
those rules. [R307-101]

I.2 The limits set forth in this AO shall not be exceeded without prior approval. [R307-401]
Status: In Compliance.
I.3 Modifications to the equipment or processes approved by this AO that could affect the emissions
covered by this AO must be reviewed and approved. [R307-401-1]
Status: In Compliance.
I.4 All records referenced in this AO or in other applicable rules, which are required to be kept by
the owner/operator, shall be made available to the Director or Director's representative upon
request, and the records shall include the two-year period prior to the date of the request. Unless
otherwise specified in this AO or in other applicable state and federal rules, records shall be kept
for a minimum of two (2) years. [R307-401-8]
Status: In Compliance.
I.5 At all times, including periods of startup, shutdown, and malfunction, owners and operators shall,
to the extent practicable, maintain and operate any equipment approved under this AO, including
associated air pollution control equipment, in a manner consistent with good air pollution control
practice for minimizing emissions. Determination of whether acceptable operating and
maintenance procedures are being used will be based on information available to the Director
which may include, but is not limited to, monitoring results, opacity observations, review of
operating and maintenance procedures, and inspection of the source. All maintenance performed
on equipment authorized by this AO shall be recorded. [R307-401-4]
Status: In Compliance.
I.6 The owner/operator shall comply with UAC R307-107. General Requirements: Breakdowns.

Status: In Compliance.

I.7 The owner/operator shall comply with UAC R307-150 Series. Emission Inventories. [R307-150]
Status: Not in compliance for additional equipment. A Notice of Intent to add a 30-kW
natural gas generator was submitted on November 7, 2019, and a new AO with equipment
and monitoring procedures identified in a previous compliance inspection (August 29,
2018), should also be incorporated into a modified AO. In addition, a NOI to add a
proposed new printing line was submitted in July of 2022. The modified AO was never
finalized due to NSR staffing changes. NSR managers have been contacted regarding the
permitting issues and a new engineer has been assigned to this project. See the attached
NSR engineering correspondence. This facility is otherwise in compliance with the current
AO conditions. No AO limits were exceeded. Records are submitted and maintained.
Emissions inventories are submitted as required. No applicable unavoidable breakdowns
have occurred since the source was last inspected. No compliance action is recommended at
this time.
Section II: SPECIAL PROVISIONS

II.A.11 Miscellaneous Maintenance Areas
Includes: small spray booth, welding areas, grinding area, etc.
Status: Out of Compliance. A Cummins 30 kW natural gas generator and a new baghouse
monitoring system has been installed at this facility. A NOI to add this equipment
was submitted on November 7, 2019. See the attached DAQ NSR email
correspondence. The permitted equipment has otherwise been installed and is
operating. II.A.7, the paint booth, is for painting frames for the lithographs and is
only occasionally used. No compliance action is recommended at this time.
II.B Requirements and Limitations
II.B.1 Site Wide Requirements
II.B.1.a Unless otherwise specified in this AO, the owner/operator shall not allow visible emissions from
any stationary point on site to exceed 10 percent opacity. [R307-401-8]
Status: In Compliance. No visible emissions were viewed from the RTO, the baghouses, or
any other point.
II.B.1.a.1 Unless otherwise specified in this AO, opacity observations of emissions from stationary sources
shall be conducted according to 40 CFR 60, Appendix A, Method 9. [R307-201-3]
Status: In Compliance. No visible emissions were observed during the inspection. See the
attached VEO.
II.B.1.b The owner/operator shall use natural gas as fuel for all stationary equipment. [R307-401-8]
Status: In Compliance. All stationary equipment is natural gas fueled. Mobile equipment
uses propane.
II.B.1.c The owner/operator shall use best working practices to reduce fugitive VOC emissions from the
facility. This shall include closing containers that hold VOC rich material, covering bins or cans
that are used to hold rags used for wiping VOC laden material, as well as other best working
practices. [R307-401-8]
Status: In Compliance. The source stores VOC laden rags in closed containers. A low VOC
propellant ink is now used. Rags are collected by Cintas and laundered appropriately for
reuse.
II.B.2 Printing Press Requirements
II.B.2.a All exhaust fumes produced by the Lithoman and Timson printing presses shall be controlled by
the regenerative thermal oxidizer before being vented to the atmosphere. [R307-401-8]
Status: In Compliance. Ducting appeared to direct printing vapors to thermal oxidizer. The
LDS Printing staff states that the duct work has not changed since the AO was issued.
II.B.2.b The regenerative thermal oxidizer shall be operated at a temperature between 1,350 - 1,700
degrees fahrenheit. [R307-401-8]
Status: In Compliance. The unit is set to operate at greater than 1500 degrees according to
mfg. recommendations. Graphs and records are kept going back to 2018. The system will
automatically shut-down if temperatures go outside of the programed range. Chamber #1
was operating at 1,451 Fahrenheit and chamber #2 was operating at 1,511 Fahrenheit at
the time of the site observations. See the attached logs for the RTO.

II.B.2.b.1 To demonstrate compliance the regenerative thermal oxidizer shall:
A. Ensure instrumentation is located such that an inspector or operator can safely read it
at any time.
B. Keep a daily computer log showing the high and low air temperatures of the exhaust
stream on a daily basis.
C. Operate with no visible emissions.
[R307-401-8]
Status: In Compliance. A control panel with a display is mounted on a wall in the southwest
corner of the Lithoman press room and is easily located and viewed. Data is recorded
electronically and available for review. No opacity was observed from the thermal oxidizer
at the time of this inspection. Temperatures are monitored and recorded continuously. See
the attached temperature graph examples.
II.B.2.b.2 A manufacture-approved preventative maintenance schedule shall be performed on the thermal
oxidizer at least once annually. [R307-401-8]
Status: In Compliance. Serviced annually. The source conducts preventive maintenance as
scheduled. The latest annual service was conducted on March 12, 2023, by Tann Corp.
II.B.2.c The VOC and HAP emissions associated with the printing press inks, paint booth paints, and
other chemicals shall not exceed the following:
32.02 tons of VOC's per rolling 12-month total
2.28 tons of combined HAP's per rolling 12-month total
[R307-401-8]
Status: In Compliance. See II.B.2.c.2 status.
II.B.2.c.1 To demonstrate compliance, the owner/operator shall calculate a 12-month rolling total for
emissions from press inks and chemicals. A new 12-month total will be calculated using data
from the previous 12 months no later than the twentieth day of each month. [R307-401-8]
Status: In Compliance. See II.B.2.c.2 status.
II.B.2.c.2 The VOC and HAP emissions shall be determined by maintaining a record of VOC and HAP
emitting inks and chemicals used each month. The records shall include the following for each
material used:
A. Name of ink or chemical being used (material)
B. Gallons or weight of material used
C. Density of material in pounds per gallon, if applicable
D. Percent by weight of all VOC and HAP components in each material
E. Amount of VOCs and HAPs emitted monthly by each material used shall be calculated
with the following equation:
VOC or HAP Emissions = (% VOC or HAP by weight) * (weight used) * (control
efficiency if applicable)

D. VOCs or HAPs from the Timson and Lithoman presses are controlled by the thermal
oxidizer and shall be accounted for assuming a 98% control efficiency
[R307-401-8]
Status: In Compliance. Reports submitted for the 12-month rolling period of September
2022, through August 2023, was reported as 2.90 tons of VOCs, and 0.02 tons of combined
HAPs. See the attached VOC/HAPs spread sheet. Calculations were based on monthly
product use totals and appear to conform to the requirements.
II.B.3 Baghouse Requirements
II.B.3.a The owner/operator shall control all paper shredding operations with the four baghouses on site.

Status: In Compliance. Ducting appeared to direct all paper dust to baghouses.
II.B.3.b The pressure drop on the baghouses shall not be less than 4.0 inches of water column or more
than 6.0 inches of water column. [R307-401-8]
Status: Out of Compliance. LDS Printing has installed a system that actually keeps a
negative pressure range between 0.0 to 1.0 inches of water pressure. The negative pressure
system also includes an airflow monitoring system that is reportedly more effective at
removing particulate. This system was referenced on the previous inspections dated July
19, 2022 (DAQC-CII04490002-22), and August 29, 2018. This company included this
monitoring system on the NOI dated November 7, 2019. The DAQ Compliance Section is
working with the assigned Minor Source NSR Engineer to have the submitted NOIs
compiled into a new modified AO. The NSR engineer has verified that LDS Printing has
submitted all of the required documents. See the attached NSR email correspondence. No
additional action is recommended at this time.
II.B.3.b.1 The pressure drop shall be monitored and recorded once daily, while the baghouses are
operating. [R307-401-8]
Status: In Compliance. Pressure drop readings are monitored and recorded daily when the
equipment is in operation. See the attached sample readings for the month of August 2023.
II.B.3.b.2 The pressure drop shall be monitored with equipment located such that an inspector/operator can
safely read the output at any time. [R307-401-8]
Status: In Compliance. Pressure drop reading instruments are situated in places that can
easily be read.
II.B.3.b.3 All instruments shall be calibrated according to the manufacturer's instructions at least once
every 12 months. [R307-401-8]
Status: In Compliance. Western States calibrates the instruments annually. The last
calibration occurred on December 8, 2022. The next calibration is scheduled for December
of 2023.
II.B.3.c Quarterly preventive maintenance records shall be kept on all four of the baghouses to insure
they are operating per the manufacture specifications. [R307-401-8]
Status: In Compliance. LDS Printing performs an in-house staff weekly/quarterly PM
maintenance. Examples of the form used for the maintenance check list is attached.

II.B.3.c.1 Maintenance records for each baghouse shall be kept to verify the quarterly preventative
maintenance evaluation was performed and detail any work that was completed. [R307-401-8]
Status: In Compliance. A NOI submitted in November 2019, requested a change in the
monitoring system description to reflect the current operation of the equipment. Western
State conducts calibrations on all the baghouses on an annual basis. The most recent
calibration was conducted on December 8, 2022. See the attached calibration certificates.
No further action is required.
II.B.4 Emergency Generator Requirements
II.B.4.a The owner/operator shall use only diesel as fuel in the emergency generator. [R307-401-8]
Status: Partial Compliance. Only diesel fuel is used in the 242 hp emergency generator
referenced as II.A.10 on the permitted equipment list. The 30-kW generator uses natural
gas. This condition description should be modified when the pending AO modification is
issued.
II.B.4.b The sulfur content of any diesel burned on site shall not exceed 15 ppm by weight. [R307-401-8]
Status: In Compliance. Thomas Pilot certifies their diesel fuel as not exceeding 15 ppm of
sulfur by weight.
II.B.4.b.1 The sulfur content shall be determined by ASTM Method D-4294-89, or approved equivalent.
Certification of fuels shall be either by source's own testing, or test reports from the fuel
marketer. [R307-401-8]
Status: In Compliance. The source purchases ULSD from Thomas Pilot which certifies
their product as meeting the federal requirements for low diesel fuels. See the attached
example invoice.
II.B.4.c Visible emissions from the emergency generator shall not exceed 20% opacity at any time.
[R307-401-8]
Status: Not Observed. Neither the diesel or the natural gas generators were operating at
the time of this inspection.
II.B.4.d The emergency generator engine shall be limited to 100 hours per rolling 12-month period for
regular maintenance, testing, and other allowed non-emergency uses listed in 40 CFR 63 Subpart
ZZZZ. There is no restriction on use during emergency situations. [R307-401-8]
Status: In Compliance. Non-emergency generator hours for the diesel fueled generator for
the time period of September 2022, through August 2023, was reported as 48.1 hours. The
non-emergency hours of operation for the natural gas generator was reported as 31.6. See
the attached generator operation hours spreadsheet.
II.B.4.d.1 Records documenting generator usage shall be kept in a log with the following information:
A. Date the generator was used
B. Duration in hours of the generator use
C. Reason for generator use
[R307-401-8]
Status: In Compliance. Generator usage logs record the dates the generators are used, the
duration of use, and indicators for times when the generators are turned on for reasons
other than testing and maintenance.

II.B.4.d.2 To determine compliance with a rolling 12-month total the owner/operator shall calculate a new
12-month total by the twentieth day of each month using data from the previous 12 months.
[R307-401-8]
Status: In Compliance. 12-month totals for September 2022, through August 2023,
indicated diesel fueled engine used for the main building operated for a total of 48.1 hours.
The natural gas emergency generator used for the Fine Arts building was reported as
operating for a total of 31.6 hours. Non-resettable hour meters have been installed on both
engines. Regular maintenance is recorded and performed. See the attached generator
records.
Section III: APPLICABLE FEDERAL REQUIREMENTS
In addition to the requirements of this AO, all applicable provisions of the following federal programs
have been found to apply to this installation. This AO in no way releases the owner or operator from any
liability for compliance with all other applicable federal, state, and local regulations including UAC
R307.
NSPS (Part 60) JJJJ: Standards of Performance for Stationary Spark Ignition Internal Combustion Engines
Status: Partial Compliance. This federal subpart applies to the Cummins 30 kW natural gas generator
that was installed in 2018. A NOI to have the engine added to a modified AO is pending review from a
DAQ NSR. To date, the hours of operation have been for maintenance and testing. Hours and
preventative maintenance are recorded as required. A non-resettable meter has been installed. See the
attached natural gas generator records. A full compliance evaluation for this engine may be pending the
incorporation of the equipment into the next modified AO.
MACT (Part 63) - ZZZZ: National Emissions Standards for Hazardous Air Pollutants for Stationary
Reciprocating Internal Combustion Engines
Status: In Compliance. Stationary RICE located at area sources of HAP emissions must comply with
Table 2d to subpart ZZZZ of MACT, which requires annual maintenance of hoses, oil, belts, filters, and
the use of ultra-low sulfur fuel. The hours of non-emergency use are under 100 annually, logs track the
engine use, and a non-resettable meter has been installed.
AREA SOURCE RULES EVALUATION:
The following Area Source Rules were evaluated during this inspection:
R307-305 Nonattainment and Maintenance Areas for PM10: Emission Standards
Status: In Compliance. This requirement is met by compliance with the opacity standards of
Conditions II.B.1.a and II.B.4.c of the AO.
Emission Standards: Sulfur Content of Fuels [R307-203]
Status: In Compliance. This requirement is met by compliance with Conditions II.B.4.b and
II.B.4.b.1 of the AO. Pilot Thomas Logistics certifies the sulfur content of the diesel fuel used.

Ozone Nonattainment and Maintenance Areas: General Standards [R307-325]
Status: In Compliance. This requirement is met by compliance with Condition II.B.1.c. All VOC
containing products are stored in sealed containers. Spills are minimized and cleaning cloths are
appropriately cleaned by Cintas.
Graphic Arts [R307-351]
Status: In Compliance. This requirement is met by Compliance with Conditions II.B.2.b, II.B.2.b.1,
and II.B.2.b.2. The source limits VOC emissions by using an add-on control consisting of a
Regenerative Thermal Oxidizer.
EMISSION INVENTORY:
An Emissions Inventory was submitted by the LDS Church Printing Center - Salt Lake Printing Center
for the 2022 Activity Year. See the attached SLEIS Summary Report. A comparison of the estimated total
potential emissions (PTE) on AO: DAQE-AN104490010-18, dated January 24, 2018, is provided.
(PTE) are supplied for supplemental purposes only.

PREVIOUS ENFORCEMENT
ACTIONS: No enforcement actions within the past five years.
COMPLIANCE STATUS &
RECOMMENDATIONS: In regards to Approval Order (AO) DAQE-AN104490010-18,
dated January 24, 2018: A NOI to add a Cummins 30 kW natural
gas standby generator to the AO was submitted to the DAQ in
2019. A new monitoring system plus a proposed new printing
line NOI was submitted in July of 2021.The modified AO to add
the equipment had not been issued at the time of this inspection
but is still pending with ongoing work between Compliance and
NSR management. See the attached DAQ engineering
correspondence. This facility was otherwise in compliance with
the current AO, MACT ZZZZ, UAC R307-203, R307-325, and
R307-351 at the time of inspection. A full compliance
determination for NSPS JJJJ for the natural gas generator is
pending the incorporation of this engine into the pending
modified AO.
HPV STATUS: Not Applicable.
RECOMMENDATION FOR
NEXT INSPECTION: This site should be targeted for an inspection after the new
modified AO adding the standby natural gas generator, new
monitoring system, and proposed new printing line has been
issued.
NSR RECOMMENDATIONS: The new AO should be issued and carefully reviewed to ensure
that all applicable conditions referencing generators and the
baghouse monitoring system have been updated, and that the
federal subpart for the natural gas generator have been
incorporated.
ATTACHMENTS: Email correspondence with DAQ Minor Source NSR, VEO,
RTO documents, VOC records, baghouse records including
calibration records, generator records, and SLEIS 2022 summary
record.

Sorry about the delay on this project. I've been unfortunately very busy lately with SIP work. The project hasn't
been cancelled. I received some additional information from the source a while ago that still needs to be
incorporated into the AO for a new monitoring system they're using for their baghouses. I will work on this project
this week and try to make progress on moving it forward. After that though, it will still need to go through all
internal reviews and a public comment period before it's issued.

Susan - After reviewing the file, this source has been out of compliance for unapproved
equipment found during inspections in 2019, 2020, 2021, and 2022. We issued a compliance
advisory/settlement agreement following the inspection in 2019 but all subsequent inspections
recommended no compliance action since an NOI was submitted in 2019. It looks like this one is
likely going to need another compliance advisory to determine if the source has been unresponsive
or if this project has gone dormant on our end.
