Dear Mr. Hansen.
In accordance with condition 1.Q.9 of Clean Harbors Aragonite's (CHA) State RCRA Permit,
this correspondence serves as written notification of a thermal relief vent opening. The thermal
relielvent opening occurred on March 2,2024.
On March 2, 2024, at approximately 3:33 PM. the facility experienced a power outage. This
caused a buildup of pressure in the system and the control board operator activated the trip
switch to open the thermal relief vent at approximately 3:34 PM. No waste was led during the
time the thermal relief vent was open. Once power was restored. the thermal relief vent was
closed at approximately 3:35 PM. It was open for approximately 57 seconds.
CHA has notified Rocky Mountain Power ol the incident and is awaiting a response on the
cause.
I certifi under penalty of law that this document and all attachments were prepared under my
direction or supervision according to a system designed to assure that qualified personnel
properly gather and evaluate the information submitted. Based on my inquiry of the person or
persons who manage the system. or those persons directly responsible for gathering the
information. the information submitted is, to the best of my knowledge and belief, true, accurate,

including the possibility of fine and imprisonment lor knowing violations.
