SUBJECT: No Further Action, Leaking Underground Storage Tank Release
Holiday Oil #1 Located at 4391 West 3500 South in West Valley City, Facility
4000377 Release OHN

A release of petroleum was reported to the Division of Environmental Response and Remediation
on March 21, 2023 based on samples taken at the time of tank closure. The extent and degree of
contamination has been fully defined and a No Further Action (NFA) review is hereby requested.

Case file information justifying no further action includes the following:

This site is an active gas station. In March 2023, two 12000-gallon gasoline underground
storage tanks (USTs) and one 12000-gallon diesel UST were removed and four new tanks were
installed. To accommodate the new tanks, the tank basin was widened. Samples taken at the
time of tank closure contained petroleum constituents above Tier 1 clean up levels.

Tank observation wells were installed in the east and west ends of the new tank basin.
Approximately two months after new tank installation, on May 26, 2023 Rockwell Solutions
collected groundwater samples from the two observation wells TB-W contained 0.015 ppm
benzene which is above Initial Screening Levels (ISLs).

Due to the lingering contamination, the site was resampled on October 25, 2023 and all
petroleum constituents were below state established cleanup levels in all sample locations.
