Dear, Julie Cobleigh
On behalf of Woodland Mutual Water Company, I am requesting an operating permit for the New
Waterline, High Tank, Booster Station Vault, PRV Vault. The previously approved Spring Improvements,
Booster Station Vault, as well as 3000 feet of 8”waterline along Hwy 35 North, were removed from the
scope of this project. Other minor changes were made that can be found in the Contract Record Drawings.
All other conditions of the plan approved by the Division of Drinking Water have been accomplished for the
water system.
The project included the installation of approximately 5 miles of new PVC Waterline replacement (DS001), a
new 325,000 gallon buried concrete storage tank, High Tank (ST003), and appurtenances. A Cemetery
Booster Vault (PF001), and Bench Creek Road PRV vault (PF002). The proposed Spring Improvements
(WS0001) were not completed due to the lack of water production from the spring in recent years. The
Spring fed the Lower Tank, and the Booster Vault (PF001) was designed in the project to boost the water to
the High Tank (ST003). Since the spring was no longer producing much water the Woodland Mutual Water
Co. elected to remove the Booster Vault (PF001) from the project. Additionally, the proposed waterline
that ran north along Highway 35 was removed from the project due to the low number of houses that
would have been served by the line. Subsequently, the houses along Highway 35 North were transferred to
the Francis Town Water System.
The Contract Record Drawings which show these changes have been provided to the Woodland Mutual
Water Company. Water samples were tested, and the results have been attached below. We have attached
the Utah Division of Drinking Water - Checklist for Issuing Operating Permits to this letter and, copy of the
Contract Record Drawings has also been sent over via e-mail. If you have any questions or need additional

Utah Division of Drinking Water ― Checklist for Issuing Operating Permits
(Per Utah Administrative Code, Rule R309-500-9)

Items 1 through 8 below must be submitted to the Division and found to be acceptable prior to operating
permit issuance. However, distribution lines without booster pumps may be placed into service prior to
submitting all of the items if a professional engineer (PE) responsible for the entire water system has been
identified to the Director and has received items 1 through 4 below. [In this case, a public water system will
submit all items needed to obtain an Operating Permit for each distribution system project after the new water lines
have been placed into service as determined by the water system’s designated PE.]
☒ 1. Certification of Rule Conformance by a PE that all conditions of Plan Approval have been
accomplished, and if applicable, changes made during construction were in conformance with
rules R309-500 through 550
I Cliff Linford P.E. No. 6311187-2202 certify that all conditions of Plan Approval were accomplished as
the project was built per plan.
☒ 2. As-built or record drawings incorporating all changes to approved plans and specifications,
unless no changes were made to the previously submitted and approved pre-construction
drawings
