Stipulation and Consent Agreement No. 9609031
Dear Mr. Reaveley:
Enclosed are the Division of Solid and Hazardous Waste (DSHW) comments which address specific
areas of the Continuing Site Investigation Plan which need to be modified. According to the
Agreement Laundry Supply Company has sixty (60) days from the date of this letter to submit a
revised Continuing Site Investigation PIan addressing the attached comments for approval.

The comments mentioned below where discussed on the phone with Mr. Mike Backe on
December 7, 1998. The following items must be addressed in writing prior to approval.
The DSHW has concern over the proper decontamination procedures used on site while drilling
the monitoring wells and while sampling the monitoring wells. Please discuss your procedure in
writing to the DSHW prior to the drilling of these wells.
The approval of Stages 3 and 4 is very premature at this time. Until collection of analytical data
from the previous proposed stages, it may be difficult to approve plans for additional wells or soil
probe locations. The approval of the off-site wells and sampling locations is dependent on the
extent of the groundwater contamination.
