Olympus Technical Services, lnc. (Olympus), on behalf of Laundry Supply Company (LSC),
submits the enclosed Report of lnvestigation for LSC's facility located at 3785 West 1987 South
Salt Lake City, Utah. The ROI documents assessment activity performed at the site during the
period May 12,1998 through January 20, 2000.
The majority of the data reported in the ROI has been previously submitted to the Utah
Department of Environmental Quality, Division of Solid and Hazardous Waste (DSHW). During
the preparation of the ROl, a reporting enor of the May 14, 1999 assessment results was
identified. Table 2, Sampling Analyses, inconectly listed the concentration of tetrachloroethene
(PCE) in the sample collected at SP-16 at 35 feet below the surface on February 27, 1999 to be
4.95 micrograms per liter (pg/l). A review of the analytical report indicates that the conect value
is 4.95 milligrams per liter or 4,950 pg/!. This enor has been corected in the ROl, and all
cunent and previously reported data have been checked for accuracy.
