Box 144880 Salt Lake City, Utah 84.114 Re: Assessment Results Laundry Supply Gompany Salt Lake City, Utah Olympus Work Order No. A7008 Dear Rob: Olympus TechnicalServices, lnc. (Olympus) performed additional site assessment activity at Laundry Supply Company's (LSC) facility located at 3785 West 1987 South Salt Lake City, Utah (Facility) in February and March 1999. The assessment included soil probe ground water sampling and analyses; and ground water monitoring well installation, sampling, and analyses. We performed the assessment in accordance with the work plan dated October 30, 1998 and revised on January 13, 1999. The Utah Department of Health and Welfare, Division of Solid and Hazardous Waste (DSHW) approved the plan on December 21,1998, subject to the inclusion of revisions submitted on January 13, 1999. The DSHW apprcved the revisions via an E-mail message dated January 21,1999. This letter report is intendd to provide the DSHW with a brief update of the assessment. A more detailed discussion of the assessment will be prepared and submitted to DSHW in the form of a site assessment report. Olympus gauged ground water elevations in Facility monitoring wells in March 1999. Figure 1 shows the location of the gauged wells. Table 1 includes a summary of the ground water elevations. The soil probe phase of the assessment included collection of ground water samples using a GeoProbe, operated by EarthProbe, at probe locations SP- 6 through SP-17 during the period February 23 through 27, 1999. Figure 1 shows the soil probe locations. Enviropro Laboratories screened the samples for tetrachloroethelene and trichloroethehne. Table 2 presents a summary of the laboratory analyses. The monitoring well phase of the assessment included the installation of wells MW.4 and MW-S by Layne Christenson on February 24 and25,1999. Figure 1 shows the monitoring wel! locations. Olympus attempted to identify a confining unit in the aquiferduring the advancement of MW.4's soil boring to 55 feet below the ground surface. We did not encounter a confining unit during the advancement of the boring. During the period March 3 through 5, 1999, Olympus developed MW..4 and IUIW-S, and sampled allof the Facility wells. Evergreen Analyticalanalyzed the samples for halogenated volatile organic (HVO) compounds. Table 2 includes a summary of the laboratory analyses. 5409lGndallSteet o Boise, lD 83706 (208) 37&5006 o Fax(208) 37&5091 o E mai!: dympusH@rmci.net C:\MA8\Prcjects\PB&L-LSC.A700ADEQ $1 4-99 sum.doc ,,^,.o, #H*ffiffid,f; utah Department of rnvironrnentar 0uarity Division of Solid and *r-r|Waste May 14, 1999 Page2 Please feel free to contact me by telephone at (208) 376-5006 or via Email at <mabolyboi@rmci.net> should you have any questions or comments.
