Shelley Executive Director DIVISION OF DRINKING WATER Nathon Lunstad Director January 29, 2024 Roy Fox Skyline Mtn Ssd 2201 SMR Fairview, Utah 84629 Dear Mr. Fox, Subject: RTCR Level 2 Assessment for Skyline Mtn Ssd Utah20043 I would like to thank you for taking the time to talk with me on the phone several times about your system’s bacteriological sampling process. Your willingness to accommodate this is greatly appreciated. Observations: This site visit was triggered by 2 Level 1 assessments in a 12-month period, Routine and Repeat TC+ EC- samples/not enough repeats, September, and October of 2023. It was however discovered the September TC+ samples were collected at Thad’s Peak filling station which has its own Well WS002, Tank ST002, and is not physically connected to the main Skyline Mtn. system. October, s TC+ Routine and repeat were from the same sample sight plan tap location connected to the main Skyline system. Conclusions: 1. System indicated September, s L1(Thad, s peak) was likely a dirty sample tap location for the routine positive sample and only 1 repeat (TC-) was collected when a total of 3 is required. October, s L1 (main system) Routine and Repeat TC+ was from the same sink tap and the system discovered a dirty screen further in the tap which is likely the cause of those unsatisfactory samples. Ground water triggered source samples were collected for both months and were TC-EC-. Roy Fox Page 2 January 29th, 2024 Recommendations: 1. Follow sample schedule when Thad’s Filling station is online adding 1 additional monthly sample ,2 total and label them accordingly. 2. Follow sanitary sampling protocol of sufficient flushing and disinfection, using a strong chlorine-based solution (applying, allowing contact time, flushing, sample collection). 3. Follow RTCR and Ground Water Rule with regards to Repeat sampling and Triggered source sampling following a routine TC+ sample. Contact DDW if not sure on follow up sampling. 4. For Thad, s Peak follow DDW seasonal Start up procedure for disinfection and flushing of system. 5.
