Subject: RTCR Level 2 Assessment Cottonwood Mutual Water Co. Users UTAH15013
I would like to thank Chris Jones and you for taking the time to meet with me and Tammie Allen
DDW on February 13th, 2024, for a Level 2 Assessment (L2) of your water system. Your
willingness to accommodate this request is greatly appreciated.
Observations:
This site visit was triggered by 2 Level 1 assessments in a 12-month period, Routine & Repeat
TC+ EC- samples, October 2023, and November of 2023.
Cottonwood Mutual water Co. is a community water system with 858 connections and a
population of 3145 customers. The system consists of 5 well sources, 6 storage tanks, and the
system does not chlorinate.
The system indicated the individual collecting the samples during these L1, s was new to the
system and was not following sanitary sampling protocols.
Conclusions:
The TC+ samples were likely sampler error as your system had a new operator performing the
sample collection and system discovered the sampler was not disinfecting the tap, not removing
the aerator and not allowing sufficient flushing to provide mainline water collection.

1. Follow RTCR and Ground Water Rule with regards to Repeat sampling and Triggered
source sampling following a routine TC+ sample. Contact DDW if not sure on follow up
sampling.
2. Follow sanitary sampling protocol of sufficient flushing and disinfection, using a strong
chlorine-based solution (applying, allowing contact time, flushing, sample collection).
3. Complete a revised RTCR sample site plan, assigning sites that are not at the end of the
system and submitting to DDW.
4. Initiate a Unidirectional system/Hydrant flushing program on a periodic basis.
5. Conduct a distribution system O & M disinfection and flushing of system, following
AWWA C-651 standards.
