(Olympus), on behalf of Laundry Supply Company (LSC), has initiated a site investigation at LSC's facility located at 3785 West 1987 South in Salt Lake City, Utah (Facility). The purpose of the investigation is to determine the nature and extent of subsurface halogenated volatile organic (HVO) compound contiamination at the Facility. This investigation is being performed in accordance with the Utah Department of Environmental Quality, Division of Solid and Hazardous Waste (DSHw)-approved Site lnvestigation Plan (SlP) (JBR EnvironmentalConsultants, lnc., November 10, 1997) and the DSHW May 6, 1998 approval of the April21, 1998 revisions to the SlP. Follonting is an interim report of investigative activities. Olympus will prepare and submit a more detailed report of the investigation at its onclusion. SOIL BORING ADVANCEUENT AND SOIL SAilIPLING On May 12 and 13, 1998, Olympus and its drilling subcontractor, Layne Christensen Company (Layne) of Salt Lake City, Utah, advaned three soil borings at the Facility. We advanced the borings to a depth of approximately 20 feet belw ground surface using a 4.2$inch inside diameter hollor stem auger. Figure 1 shors the locations of the soil hrings labeled SB-1, SB-2, and S&3. Layne and Olympus collected soil samples using a split spoon soil sampler at five'foot intervals during the advancement of the soil borings. Olympus screened the soil samples for tetrachloroethene using Drager gas detector tubes and logged the samples for soil classification purposes. Attacfiment A indudes copies of the Soil Boring Logs. Soil cuttings generated during the boring program were drummed and are presently stored at the Facility pending disposal anangements. (Evergreen) of \A/heat Ridge, Colorado. At Olympus' instruction, Evergreen composited (by weight) the sample set from each boring into one sample for analyses. ln addition, the laboratory prepared grab samples for analyses, Iabeled SB2-1.5 and SB&3, from split spoon samples collected immediately above the watertable from S&2 and SB-3, respectively. Evergreen analyzed the samples for halogenated HVO compounds using procedures documented in SW-846 Method 8260. Table 1 presents a summary of the soil sample analyses. Attachment B includes copies of the laboratory analytical reports, the Sample Chain of Custody Record, and Olympus'data validation report. MONTTOR|NG rmELL COMPLETTONS AND GROUND WATER SAI'|PLfNG Olympus and Layne completed the three soil borings as ground water monitoring wells labeled MW-1, MW-2, and MW-3 (Figure 1). We completed the wells as approximately 20-foot deep two-inch diameter PVC wells. Attachment A includes copies of the Monitoring Well Completion Logs. On May 14, 1998, Olympus surveyed the monitoring wells for vertical and horizontal control to a relative Facility benchmark (100 feet) and gauged ground water elevations in the wells. Table 2 presents a summary of the ground water elevation gauging. Olympus prepared a Facility ground water elevation contour map based upon the May 14, 1998 ground water elevations (Figure 1). Based upon the data used to compile the map, we calculated a ground water flow direction at the Facility to the north-northwest with a gradient of 0.02 feet per feet. ln preparation for ground water sampling, Olympus developed the monitoring wells using an airlift pump on May 14, 1998. We sampled the wells on May 15, 1998. Prior to sampling, we purged the wells of three casing water volumes using disposable polyethylene bailerc. The wells were sampled immediately after purging using the same bailer. For quality assuran@ and quality contro! (OA/OC) purposes, Olympus prepared an equipment blank (EB), a trip blank (l'B), and a duplicate sample from MW-2 (labeled MW-4). As part of our ground water monitoring program, Olympus measures pH, electricalconductivity, dissolved oxygen, and temperature (field pararneters) at each monitoring well. Table 2 includes a summary of the field parameter measurements. Olympus shipped the ground water and QA/QC samples by ovemight courier, using chain of custody protocol, to Evergreen Analytical, lnc. (Evergreen) of Wheat Ridge, Colorado. The laboratory analyzed the samples for HVO compounds using prccedures documented in SW- 846 Method 8260. Table 3 presents a summary of the ground water sample analyses. Attachment B indudes copies of the laboratory analytical reports, the Sample Chain of Custody Record, and Olympus'data validation report. Olympus drummed all development and purge water generated during the ground water monitoring event. The water storage drums are presently stored at the Facility pending disposal anangements. d:tlsc.7585Beq &1 998 ir.doc t HazaUtah Division of Solid and June 19, 1998 Page 3 rdous Waste ADDITIONAL ASSESSMENT ln an effort to befter define the nature and extent of HVO compounds in ground water at the Facility, Olympus proposes to continue the assessment dorrn-gradient of the release area. We propose to continue the assessment using soil probes outfifted with ground water sampling apparatus. EarthProbe of Salt Lake City, Utah willprovide soil-probing services. EarthProbe willdrive five soil probes at apprcximate locations shown in Figure 1. We willgauge ground water elevations and calculate ground water flow direction prior to soil probing activities to confirm the flow direction. The soil probe locations will be moved (if necessary) to better locate the probes down-gradient of the HVO release area (abandoned tank basin) based on the calculated ground water flor direction. At soil probe locations SP:l through SP-S, EarthProbe and Olympus willcollecl ground water samples at depths of 15, 25 and 35 feet belovv the ground surface. Olympus will deliver the water samples in appropriate contrainers, using chain of custody protocol, to OnSite Analytical Services, lnc. (OnSite) of Salt Lake City, Utah for screening analyses. The laboratory wil! screen the samples for HVO compounds using a proprietary analytical method. One duplicate sample from the event will be shipped to Evergreen for confirfnation analyses. Evergreen will analyze the confirmation sample using protocol established in SW-846 Method 8260. Following sample collection, EarthProbe, willabandon the probe holes by sealing them grout. EarthProbe will decontaminate all down-hole equipment with a pressure washer. Decontamination waterwill be contained in drums and stored at the Facility pending disposal anangements. SCHEDULE The above-described soil probing and ground water sampling is scheduled forJune 26, 1998.
