Attention: Izola Vincent, President and Registered Agent 1230 West 1400 North Lehi, Utah 84043 Re: Additional Sampling Requirements and Vapor Sampling Plan Former Calvinco, Co., UST, located at 3120 South Main Street, South Salt Lake City, Utah Facility Identification No. 4000124, Release Site OBL Dear Ms. Vincent: The Division of Environmental Response and Remediation (DERR) completed its review of Calvinco Company’s (CC) Soil Vapor Sampling proposal letter, submitted to the DERR on January 3, 2024, by Underwood Environmental Consulting, Inc. (UEC), for the above-referenced facility. The CC Soil Vapor Sampling proposal letter indicates that CC was unable to secure an access agreement to perform sub-slab vapor testing in the Intermountain Lock and Security Supply (IML) building due to concerns for the concrete foundation, nearby utilities, and the perceived risks identified by IML. According to the CC Soil Vapor Sampling proposal letter, IML representatives claim there have never been complaints of hydrocarbon odors inside the building. In lieu of the in-building sub-slab testing near MW-7, the DERR project manager suggests a sub-slab testing point outside of the building placed in a concrete slab, near the south building entrance, be considered; however, sub-slab testing near MW-5 will still be required. The revised soil vapor sampling plan should include renderings of the building’s internal layout at ground level, the type of work space present, the layout of the parking garage and the nearby subsurface utilities, presented as problematic to the tests. The air sample analysis must be performed by an approved laboratory for the MBTEXN and TPH by Method TO-15. In accordance with Utah Code R311-202, the DERR requires that CC provide a soil-vapor testing plan within 90 days of receipt of this letter.
