Your Waste Tire Transporter Registration Application has been reviewed and administrative approval for registration as a waste tire transporter is hereby granted. Champlin Tire Recycling,
Inc.has been assigned the Solid Waste Tire TransporterNumber TR2024-02.Champlin Tire Recycling, Inc.shoulduse this number on all future correspondence sent to the Division of Waste Management
and RadiationControl (Division) regarding transporting of waste tires in the state of Utah.
This registration as a waste tire transporter will be valid until December 31, 2024, by which time a new registration must be filed with the Division.
This approval is required to comply withrequirements of the Waste Tire Recycling Act in the state of Utah. Maintenance of a valid registration and full compliance with all applicable
requirements for a waste tire transporter does not relieve waste tire transporters of the responsibility to obtain and comply with any other required local, state, or federal approvals
or permits. This registration may be revoked in accordance with Utah Administrative Code R315-320-5(9).
