Your Waste Tire Transporter Registration Application has been reviewed and administrative approval for registration as a waste tire transporter is hereby granted. Liberty Tire Services
of Ohio, LLChas been assigned the solid waste tire transporter number TR2024-01.Liberty Tire Services of Ohio, LLC should use this number on all future correspondence sent to the Division
regarding transportation of waste tires in the state of Utah.
This registration as a waste tire transporter will be valid until December 31, 2024 by which time a new registration must be filed with the Division.
This approval is required in order to be in compliance with requirements for the transportation of waste tires in the state of Utah. Maintenance of a valid registration and full compliance
with all applicable requirements for a waste tire transporter does not relieve waste tire transporters of the responsibility to obtain and comply with any other required local, state,
or federal approvals or permits. This registration may be revoked in accordance with Utah Administrative Code R315-320-4(10).
