Dear McKay Quinn:
The Closure Plan for the above-referenced facility, received by the Division of
Environmental Response and Remediation (DERR) on December 13, 2023, has been approved
subject to the noted modifications, if any. The DERR office must be notified three business days
before beginning closure activities. The local fire department and other local agencies may have
additional requirements for closure or may charge inspection fees.
The Closure Plan Approval is effective for a period of one year from the date of approval.
If closure does not take place within one year, submittal of a new Closure Plan will be required,
unless otherwise approved by the Division Director.
Any change to the approved Closure Plan must be submitted, in writing, and approved by
the Division Director before implementation. If contamination is suspected or found during closure
activities, you must report it to the DERR at (801) 536-4100 within 24 hours of discovery.
Your closure plan indicates soil aeration as a possibility for remediation in the event
contamination is found. If soil aeration is chosen, all requirements of the DERR regarding
Corrective Action Plans must be met prior to the commencement of any aeration activities.
Contaminated soils must be safely stockpiled on site until approval for aeration is granted. Please
contact the DERR if you have any questions.
The three (3) tanks must be removed by May 1, 2024. If the tanks are not removed the
facility will be subject to appropriate compliance actions and penalties.

Enclosed is a copy of the Closure Notice form, which must be completed and submitted to
the Division Director after the closure is performed. Please submit the environmental and Unified
Soil Classification sample analysis data and chain-of-custody forms with the Closure Notice as soon
as possible, but no later than 90 days after tank closure. Also, a final Site Plat that meets the
requirements outlined in R311-204-4(2)(b) must be submitted with the Closure Notice.

SITE ASSESSMENT
A site assessment must be perfonned for all PST closures and change-in-service . Site assessments must be perfonned as outlined in 40 CFR
280.72 and U.A.C . R31 l-205. lf contamination is suspected, additional samples must be collected at the location where contamination is
most likely to be present. If groundwater is encountered , a soil sample must be collected, in the unsaturated zone, in addition to each
groundwater sample . Soil and groundwater samples must be analyzed for the compounds shown in the following table, using appropriate lab
methods .

CONTAMINATED MATERIALS MUST BE DISPOSED AT AN ACCEPTABLE FACILITY:
All materials generated from PST closures must be managed and disposed in a manner that does not place those materials in direct contact
with the environment . On-site stockpiling of contaminated soils may be required prior to any soil management activities. Any person
[)_rovidinf! remedial assistance for a fee , inc/udinJl aeration and over-excm,ation (of more than 50 ycl3), must be a Certified UST C onsultant.

CONTAMINATION INFORMATION
If contamination at the facility is suspected or confirmeg, the infonnation must be reported to the DERR Division Director at (801) 536-4100
within 24 hours . The Division of Water Quality must be notified at (801) 536-4300 if Free Product is encountered or if surface water has
been impacted. If contamination is confirmed , any person assisting in the remediation process for a fee must be a Certified UST Consultant.
