Subject: Notice of Increased Monitoring for Fruitland Water SSD, UTAH07058
Bonner Hardgree:
According to the Division of Drinking Water’s (the Division) records, the Mill Hollow Upper
Spring (listed as WS001 in the Divisions database) was determined to have exceeded the turbidity
maximum contaminant level (MCL) on June 28, 2022. The MCL for turbidity is 5 Nephelometric
Turbidity Units (NTU), and the combined average of the four (4) measurements taken per the
requirements of R309-205-8(2) was 10 NTU. As a result, the Mill Hollow Upper Spring (WS001)
was required to treat for turbidity, per the letter attached here from June 28, 2022.
Fruitland Water SSD (UTAH07058) redeveloped the Mill Hollow Upper Spring (WS001) and
received a temporary operating permit for the redevelopment on January 16, 2024. The temporary
operating permit contained requirements for bacteriological monitoring and Microscopic
Particulate Analysis (MPA) testing. Since the Mill Hollow Upper Spring (WS001) was also
determined to be required to treat for turbidity prior to the redevelopment, the source is also
subject to increased monitoring for turbidity to determine the success of the redevelopment.
Fruitland Water SSD is required to take monthly turbidity samples at Mill Hollow Upper Spring
(WS001) during the operating period when the spring is accessible and shall continue for a
minimum of 12 months of data. This schedule will begin in March 2024. If there are months the
spring is not accessible, you are required to notify the Division.
If at any point the turbidity of the Mill Hollow Upper Spring (WS001) is measured above 5 NTU,
Fruitland Water SSD shall take three (3) additional follow up turbidity samples from the spring
within a month of the initial sample date.

Subject:Turbidity MCL Exceedance and Violation,Treatment Required –
Fruitland Water SSD UTAH07058, Mill Hollow Upper Spring (WS001)
Bonner Hardgree:
The Division of Drinking Water (the Division) records indicate that you are the responsible party
for Fruitland Water SSD (UTAH07058). The Fruitland Water SSD water system is a public water
system and thus subject to the Administrative Rules for Public Drinking Water Systems.
On May 3, 2022, Fruitland Water SSD measured turbidity from Mill Hollow Upper Spring in
response to positive bacteriological test results. A result of 20.8 Nephelometric Turbidity units
(NTUs) was obtained, significantly above the 5 NTU Maximum Contaminant Limit (MCL)
(R309-200-5(5)(c)(ii)). Fruitland Water SSD measured turbidity from Mill Hollow Upper Spring
three additional times, per the requirements of R309-205-8(2). The combined average of these
four measurements is 10.07 NTU, which is above the turbidity MCL of 5 NTU and results in a
turbidity MCL violation being assessed to Mill Hollow Upper Spring (WS001).Per
R309-205-8(2), Mill Hollow Upper Spring (WS001) is reclassified as being required to treat
for turbidity.
The test results for Mill Hollow Upper Spring (WS001) show iron levels greater than the EPA’s
secondary MCL of 0.3 mg/L (R309-200-6). Secondary MCLs represent concentrations above
which aesthetic impacts to the drinking water may be observed. Mill Hollow Upper Spring is
consistently producing water that exceeds secondary MCLs for iron, likely contributing to the
high turbidity observed in Mill Hollow Upper Spring.
Required Actions
Tier 2 public notice is required for this turbidity MCL violation, with public notice required
for as long as Mill Hollow Upper Spring is in use and exceeds the turbidity MCL.The public
notice template is attached. The system must provide the public notice as soon as practical, but no
later than 30 days after the system learns of the violation. If the public notice is posted, the notice
must remain in place for as long as the violation persists, but in no case for less than seven days,

even if the violation is resolved (see R309-220-6(1)(a) and R309-220-6(1)(b)). This public notice
requirement cannot be waived once a violation occurs. Proof of public notification must be
provided to the Division at ddwreports@utah.gov within 30 days of notifying the water system
customers and resubmitted each quarter the violation remains unresolved.
Prior to using Mill Hollow Upper Spring (WS001) as a drinking water source, you must
install and operate a Division-approved permanent treatment facility to remove turbidity or
complete successful redevelopment of the source. Mill Hollow Upper Spring will be marked
as an unapproved source until action is taken. If turning this source out of the system is not
an option, Fruitland Water SSD may continue to use the source, but 200 IPS points will be
assessed for use of an unapproved source, and public notice will be required on an ongoing
basis as outlined above.
Installation of a permanent turbidity treatment facility must go through the Division’s approval
process including:
1.Complete Division Plan Approval prior to construction of a treatment facility.
2.Issuance of a Division Operating Permit for the treatment facility prior to operations.
Mill Hollow Upper Spring will remain an unapproved source until permanent treatment is
installed to remove turbidity or the source is successfully redeveloped.This source has already
been designated as an unapproved source due to the requirement to disinfect to secondary
standards, so no additional points will be assessed. It is the Division’s understanding that Fruitland
Water SSD intends to enter into a Corrective Action Plan (CAP) to provide the time needed to
redevelop this source in an attempt to eliminate the bacteriological and turbidity issues requiring
treatment. After redevelopment, a period of source sampling for both bacteriological and turbidity
parameters will be required to demonstrate successful redevelopment.
Additional Monitoring
Based on the determination that Mill Hollow Upper Spring (WS001) is required to treat for
turbidity, the following monitoring is required:
1.Water Quality Sampling
a.This system is required to take monthly samples at Mill Hollow Upper Spring
(WS001)for turbidity and iron during the operating period.This schedule will
begin July 2022.Additional monitoring may be required as a condition of the
CAP.
b.Data should be sent directly to Sarah Page at sepage@utah.gov by the 10th of the
following month.
