The file for this release has been reviewed by the Division of Environmental Response and
Remediation (DERR) project manager who has recommended that no further corrective action be
taken at this time. This no further action applies only to the specific Release Site NBQ. This
recommendation is based on the information contained in the file for this release . That information
was supplied by you, your consultant, or another representative.
The information submitted indicates that petroleum contamination remains from this
release . Specifically, there is soil remaining that exceeds the DERR's Initial Screening Level (ISL)
cleanup standard, in the former underground storage tank (UST) location (circa 2013) and near the
south boundary of the site . Groundwater remaining that exceeds the ISL is found at the former UST
location and areas northwest (see enclosed Remaining Contamination Above the ISL Standard
Map).
Due to the nature and location of contamination that has been left in-place, the DERR is
utilizing Blue Stakes of Utah to receive notification when excavators may be digging near the
contaminated area. The DERR will provide the attached flyer to excavators when notification is
received from Blue Stake (see enclosed Notification of Subsurface Petroleum Contamination).
The contamination is at depths and locations that are not currently considered a threat to
human health or the environment according to Utah Admin. Code R311-21 l. This evaluation is
based on current land usage, exposure pathways, potential receptors and other site-specific risk

vicinity of the above referenced property.

Maverik, #481, 1874 West 2700 North, Farr West, Utah
The Maverik, #481supports a convenience store with fueling operations on a corner location in Farr West,
Utah. In September 2013, a petroleum release was reported to the Division of Environmental Response and
Remediation (DERR) based on the results of an underground storage tank (UST) closure. The DERR
assigned NBQ .
Over eight-years of site investigations and groundwater monitoring has occurred, with no corrective actions
to remedy site contamination, however, attenuation of soil and groundwater contamination has been
demonstrated. Remnants of soil and groundwater contamination exist on-site, and near a southern utility
corridor, with analytic results above for gasoline.
On December 19, 2023, a site closure review by the DERR Closure Committee occurred, determined the site
meets the Tier I standard coupled with an on-site remaining contamination map, and notification to the Blue
Stakes of Utah network. NFA is requested.
