INCIDENT SUMMARY
Caller reported that an engineering firm, Landmark Engineering, was investigating a septic system failure at the facility, when the collected soil
samples were noted with a heavy fuel odor. The Southeast Utah Health Department (SEHD) investigated and noted the diesel dispenser was
malfunctioning causing minor leaking to the ground. The SEHD conducted a subsequent visit to the site and found significant quantities of diesel and
gasoline had released from the dispensers and entered into a dry wash adjacent to the property. The facility has above ground storage AST tanks with
secondary containment that appeared to be in good condition.
