Subject: Source Reactivation Requirements for Clearfield City Water System; UTAH06002
Kenny England:
On January 12, 2023 you notified the Division of Drinking Water (the Division) that the
Reservoir Well (identified as WS001) is an active source that Clearfield City Water System uses
to supplement the water purchased from Weber Basin WCD (UTAH29023) in the summer
months. This well is currently listed as inactive in the Divisions database. Before the well can be
formally reactivated and utilized as a source of drinking water, there are several items which
need to be completed.
Permitting Requirements
The Division's records indicate this source was taken offline due to the pump motor being
inoperable. Further, we have it documented that the well is equipped with a 1,485 gallon per
minute well pump. If Clearfield installed a new motor and pump, please send in a project
notification form, pump specifications, and new capacities for the wells
to ddwpnf@utah.gov within 120 days of receiving this letter. If the motor was able to be
repaired, please contact Dani Zebelean at (385) 278-5110 (dzebelean@utah.gov).
Compliance and Operator Assistance Requirements
On October 2, 2023, Clearfield City Water System was given a sanitary survey
and six significant deficiencies were found during that survey, including one significant
deficiency involving Reservoir Well (WS001). The system was given 120 days or until February
10, 2024 to correct these deficiencies. If corrections have been made, please send in a description
on how it was corrected and pictures to ddwips@utah.gov. If these have not been fixed and you
need more time, please go to requestcap.utah.gov to request a Corrective Action Plan to allow
more time to fix these issues.

Source Monitoring Requirements
The following samples must be collected prior to utilizing Reservoir Well (WS001). The routine
monitoring frequencies for each are different and will be determined using the results of the
listed analyses in accordance with applicable portions of R209-205.
