Subject: Notice of Facility ID Change, Monitoring Requirements Remain the Same, Three
Points Center: UTAH27088

This letter is to notify Three Points Center (“the system” UTAH27088) that the reported sulfate
levels present in the Academy Well (identified as WS001 in the Division of Drinking Water’s (the
Division) database) exceed the Maximum Contaminant Level (MCL) of 1000mg/L. The running
annual average (RAA) of Sulfate at the Academy Well is 1035mg/L. According to the Division’s
records, the Three Points Center operates a reverse osmosis treatment plant (identified as TP001)
which treats the Academy Well (WS001) prior to the first connection served.
To determine compliance with the Sulfate standard, the Division requests that the system begin to
take quarterly Sulfate Sodium and TDS samples immediately downstream of the reverse osmosis
treatment plant (TP001). This requirement replaces the quarterly samples collected from the
Academy Well (WS001). The systems quantity of required samples remains the same, however
the Division requests that the Sulfate Sodium and TDS samples be pulled downstream of the
treatment plant instead of before.
