Henrieville Town Water System: UTAH09006

This letter is to notify Henrieville Town Water System (UTAH09006) that following a Sanitary
Survey conducted October 2, 2023, the Division of Drinking Water (the Division) has merged the
South Horizontal Well (listed as WS004 in the Divisions database) and North Horizontal Well
(WS005) into a new combined spring facility named North and South Horizontal Well Spring.
The facility ID for the new facility is WS007.

Source samples are to be collected at the point of entry into the distribution system, after the water
has undergone any treatment and before the first connection is served. For this reason, the location
at which Henrieville Town collects their routine source samples has also been modified. To
capture the water quality of the North and South Horizontal Well Spring (WS007) after it has
undergone both chlorination and filtration, please collect all source samples at the effluent sample
tap in the Iron Removal Building (TP004). Please indicate the facility ID as TP004 on all
laboratory paperwork for samples taken at this location.

These changes are available to view anytime online on your systems water monitoring page on
waterlink.utah.gov.
