The decommissioning activities involved the proper abandonment of nine (MW -1, 2, 5, 6, 10, 11, 12, 13, and 14) two-inch diameter groundwater monitoring wells located at the site. Since the wells were over 30 feet deep, a state of Utah licensed well driller was required to decommission the wells. ConeTec, Inc. (ConeTec) performed the decommissioning activities with oversite from Atlas Technical Consultants (Atlas). A map showing the site layout and the decommissioned well locations is presented as Figure 1. The following activities were completed during the decommissioning. Well casings were backfilled to approximately 12 inches below grade using a grout mixture containing bentonite and cement. The well casings were broken off approximately 5-inches below grade. The flush mount sleeves remained in place and were sealed with concrete. The drilling subcontractor was responsible for disposal of all materials excavated and removed during decommissioning. There was such a small quantity that the materials were placed in th e on-site solid waste receptacle. Photographs 1 through 12 show the various stages of the decommissioning process and finished results.
